摘要
目的 观察助产士一对一导乐陪伴分娩在初产妇无痛分娩中的应用效果.方法 选取2022年8 月至2023年8月在聊城市第二人民医院分娩的90例初产妇为研究对象,按照随机数字表法分为两组,每组45例.对照组实施常规无痛分娩干预,观察组在对照组基础上进行助产士一对一导乐陪伴分娩,比较两组的分娩方式、产程、分娩自我效能感、炎症与应激指标、母婴结局.结果 观察组阴道分娩率高于对照组,差异有统计学意义(P<0.05);观察组第一、二、三产程均短于对照组,差异有统计学意义(P<0.05);干预后,两组分娩自我效能感量表(CBSEI-C32)评分升高,且观察组CBSEI-C32 评分高于对照组,差异有统计学意义(P<0.05);干预后,两组C反应蛋白(CRP)、肾上腺素(AD)、去甲肾上腺素(NE)水平均降低,且观察组CRP、AD、NE水平低于对照组,差异有统计学意义(P<0.05);观察组不良母婴结局总发生率低于对照组,差异有统计学意义(P<0.05).结论 助产士一对一导乐陪伴分娩在初产妇无痛分娩中的应用效果显著,值得推广.
Abstract
Objective To observe the application effect of one-to-one Doula accompanied delivery by midwives in the painless delivery of primiparas.Methods A total of 90 primiparas who gave birth in the Second People's Hospital of Liaocheng from August 2022 to August 2023 were selected and and divided into two groups by the random number table method,with 45 cases in each group.The control group was given routine painless delivery intervention,while the observation group was given one-to-one Doula accompanied delivery by midwives on the basis of control group.The delivery mode,labor process,childbirth self-efficacy,inflammatory and stress indicators,and maternal and infant outcomes were compared between the two groups.Results The vaginal delivery rate of observation group was higher than that of control group,the difference was statistically significant(P<0.05).The first,second,and third stages of labor in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).After intervention,the score of short form of the Chinese childbirth self-efficacy inventory(CBSEI-C32)was increased in both groups,and the score of CBSEI-C32 in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).After intervention,the levels of C-reactive protein(CRP),adrenaline(AD),and norepinephrine(NE)in both groups were decreased,and the levels of CRP,AD,and NE in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion One-to-one Doula accompanied delivery by midwives in the painless delivery of primiparas has significant application effect,which is worthy of promotion.
