摘要

目的 分析极早产儿出生后1周发生低磷血症的危险因素.方法 选取2020年1月至2021年12月安徽医科大学第一附属医院收治的79例极早产儿为研究对象,根据出生后1周是否出现低磷血症分为低磷血症组(n=36)及血磷正常组(n=43).采用单因素、多因素logistic回归分析发生低磷血症的危险因素,并采用受试者工作特征(ROC)曲线分析相关指标对低磷血症的早期预测价值.结果 单因素分析结果显示,低血磷症组出生体重低于血磷正常组(P=0.001),低血磷症组妊娠高血压患病率高于血磷正常组(P=0.044),低血磷症组胎膜早破>18 h占比高于血磷正常组(P=0.002),低血磷症组出生后第7天血清肌酐水平低于血磷正常组(P=0.034);多因素分析结果显示,出生体重是极早产儿出生后1周发生低磷血症的保护因素(P<0.05);ROC曲线分析结果显示,出生体重预测低磷血症的曲线下面积为0.893,灵敏度为0.767,特异度为0.917.结论 出生体重越低,极早产儿出生后1周内发生低磷血症的可能性越高.

Abstract

Objective To analyze the risk factors for hypophosphatemia at 1 week after birth in extremely premature infants.Methods A total of 79 extremely premature infants admitted to the First Affiliated Hospital of Anhui Medical University from January 2020 to December 2021 were selected as the research objects.According to the presence or absence of hypophosphatemia at 1 week after birth,they were divided into hypophosphatemia group(n=36)and normal serum phosphorus group(n=43).Univariate and multivariate logistic regression analysis were used to analyze the risk factors of hypophosphatemia,and the receiver operating characteristic(ROC)curve was used to analyze the early predictive value of related indicators for hypophosphatemia.Results Univariate analysis showed that the birth weight of hypophosphatemia group was lower than that of normal phosphorus group(P=0.001).The prevalence rate of gestational hypertension in the hypophosphatemia group was higher than that in the normal phosphorus group(P=0.044).The proportion of premature rupture of membranes>18 h in the hypophosphatemia group was higher than that in the normal phosphorus group(P=0.002).The serum creatinine at 7 days after birth level of hypophosphatemia group was lower than that of normal phosphatemia group(P=0.034).The multivariate analysis showed that birth weight was a protective factor for hypophosphatemia at 1 week after birth in extremely premature infants(P<0.05).ROC curve analysis showed that the area under the curve of birth weight predicting hypophosphatemia was 0.893,the sensitivity was 0.767,and the specificity was 0.917.Conclusion The lower the birth weight,the higher the likelihood of hypophosphatemia within 1 week of birth of extremely premature infants.
