摘要
目的 分析子宫内膜息肉(EP)患者实施宫腔镜下电切术联合左炔诺孕酮宫内缓释节育系统的治疗效果及对月经失血图(PBAC)的影响.方法 对江阴市人民医院 2020 年1月至 2022 年12 月收治的150 例EP患者展开研究,应用抽签法将其分为研究组与对照组,各 75 例.所有患者均采取宫腔镜下电切术治疗,对照组术后给予黄体酮胶囊,研究组术后放置左炔诺孕酮宫内缓释节育系统.比较两组PBAC评分、子宫内膜厚度,以及术后12 个月的复发情况.结果 两组术后1、3、6、12 个月PBAC评分低于术前(P<0.05);对照组术后 3、6、12 个月PBAC评分较术后1个月逐渐升高(P<0.05);研究组术后 3、6、12 个月PBAC评分低于对照组(P<0.05).两组术后1、3、6、12 个月子宫内膜厚度小于术前(P<0.05);对照组术后 6、12 个月子宫内膜厚度较术后1个月增厚(P<0.05);研究组术后 3、6、12 个月子宫内膜厚度小于对照组(P<0.05).研究组复发率低于对照组(P<0.05).结论 EP患者实施宫腔镜下电切术联合左炔诺孕酮宫内缓释节育系统能够减少月经量及降低子宫内膜厚度、EP复发率,远期效果更佳.
Abstract
Objective To analyze the therapeutic effect of hysteroscopic electrotomy combined with Levonorgestrel Releasing Intrauterine System on patients with endometrial polyp(EP)and its effect on pictorial blood loss assessment chart(PBAC).Methods A total of 150 EP patients admitted to Jiangyin People's Hospital from January 2020 to December 2022 were studied,and they were divided into study group and control group by lottery method,with 75 cases in each group.All patients were treated with hysteroscopic electrotomy.The control group was given Progesterone Capsules after operation,and the study group was placed with Levonorgestrel Releasing Intrauterine System after operation.The PBAC score,endometrial thickness,and the recurrence rate 12 months after surgery were compared between the two groups.Results The PBAC scores of the two groups at 1,3,6 and 12 months after operation were lower than those before operation(P<0.05).In the control group,the PBAC scores at 3,6,and 12 months after operation were gradually higher than those at 1 month after operation(P<0.05).The PBAC scores of the study group were lower than those of the control group at 3,6 and 12 months after operation(P<0.05).The endometrial thickness of the two groups at 1,3,6 and 12 months after operation was thinner than that before operation(P<0.05).In the control group,the endometrial thickness at 6 and 12 months after operation was thicker than that at 1 month after operation(P<0.05).The endometrial thickness of the study group was thinner than that of the control group at 3,6 and 12 months after operation(P<0.05).The recurrence rate of the study group was lower than that of the control group(P<0.05).Conclusion Hysteroscopic electrotomy combined with Levonorgestrel Releasing Intrauterine System in patients with EP can reduce menstrual volume,endometrial thickness,and the recurrence rate of EP,and the long-term effect is better.
