目的:探究慢性阻塞性肺疾病急性加重期(AECOPD)患者不同炎症表型对肺功能的影响.方法:选取2019年2月~2022年8月在合肥京东方医院进行治疗的124例AECOPD患者作为研究对象,将嗜酸性粒细胞(EOS)比例<3%作为非EOS组,EOS比例≥3%作为EOS组.检测两组患者的肺功能分级、肺一氧化碳弥散量(DLCO%)、第一秒用力呼气容积与用力肺活量比值(FEV1/FVC)、用力肺活量(FVC)、第一秒用力呼气容积(FEV1)、肺活量(VC)及深吸气量(IC).对比两组患者的肺功能,应用单因素及多因素Logistic回归分析探讨AECOPD患者肺功能减损的影响因素.结果:非EOS组患者肺功能等级优于 EOS 组(P<0.05).EOS 组患者的 DLCO%、FEV1/FVC、FVC、FEV,、VC 及 IC 低于非 EOS 组(P<0.05).EOS 型是 AECOPD患者肺功能减损的危险因素(P<0.05).结论:炎症表型会影响AECOPD患者的肺功能,EOS型是AECOPD患者肺功能减损的影响因素.
Effects of different inflammatory phenotypes on the lung function in patients with acute exacerbation of chronic obstructive pulmonary disease
Objective:To investigate the effects of different inflammatory phenotypes on the lung function of patients with acute exacerbation of chronic obstructive pulmonary diseases(AECOPD).Methods:A total of 124 patients of AECOPD treated in our hospital were included from February 2019 to August 2022,and assigned to non-EOS group(patients with a proportion of eosinophils,EOS<3%)and EOS group(patients with EOS≥3%).Patients in both groups underwent pulmonary function classification,measurement of diffusing capacity of pulmonary carbon monoxide dispersion(DLCO%),ratio of forced expiratory volume in the first second to forced vital capacity FEV1/FVC,forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),vital capacity(VC)and deep inspiratory capacity(IC).The lung function of the two groups was compared,and univariate and multivariate logistic regression analysis was performed to observe the factors involved in lung function impairment in the AECOPD patients.Results:Patients in non-EOS group had better lung function level than those in EOS group(P<0.05),and DLCO%,FEV1/FVC,FVC,FEV1,VC and IC in EOS group were lower than those in non-EOS group(P<0.05).EOS type was a risk factor for lung function impairment in AECOPD patients(P<O.05).Conclusion:Inflammatory phenotype can affect the lung function of AECOPD patients,and EOS type is an influential factor for the lung function impairment in patients with AECOPD patients.
