Survey on the self-management ability in type 2 diabetes patients under community chronic disease management mode
Objective:To understand the self-management ability and related influencing factors in type 2 diabetes patients under community chronic disease management mode,so as to provide reference for further exploring the intervention model for self-management behavior.Methods:One hundred and sixty-four type 2 diabetes patients were recruited from two community health service centers in Hefei by convenient sampling method,and were face-to-face surveyed with a self-designed questionnaire that consisted of patients'basic information,self-management behavior and self-empowerment ability.Results:The total score of self-management behavior was(44.85±9.01).Scoring on each component from high to low were medication management(5.48±1.58),diet management(4.83±0.96),exercise management(4.66±1.85),foot management(2.88±1.13)and blood glucose monitoring(2.49±1.51).Univariate and multivariate analysis showed that the patients living with their spouse,having a family doctor and scoring high in self-empowerment ability,had higher level of self-management ability(P<0.05).Conclusion:More effective community chronic disease management mode can be explored through measures such as improving the quality of contract service with the family doctor,so as to further help type 2 diabetes patients improve their self-management ability and promote health improvement.
