Analysis on the high risk factors for the liver damage in children with Mycoplasma pneumoniae pneumonia
Objective:To investigate the high risk factors for liver damage in children with Mycoplasma pneumoniae pneumonia(MPP).Methods:One hundred and seven children with MPP complicated with liver damage treated in Linquan County People's Hospital between January 2020 and August 2022 were included as study subjects,and another 110 children with MPP without liver damage,treated on hospital basis in the same period,were included as controls.The two groups were compared regarding the general information,indicators in laboratory examination and imaging findings.Both univariate and multivariate analyses were performed to analyze the high risk factors for children with MPP complicated with liver damage,and the optimal cut-off values were determined by receiver operating characteristics(ROC)curve analysis to obtain the early prediction indicators for children with MPP complicated with liver damage.Results:Univariate analysis showed that the two groups were significantly different in the duration of hospital stay,heat duration,heat peak(≥39.0℃),the days of hormone use,LYM,levels of RDW,Hb,CRP,ALP,TP,ALB,LDH,CKMB,α-HBDH,lung compaction,and lesion involvement of lung lobe≥2(P<0.05).Multivariate logistic regression analysis showed that longer heat course,higher heat peak≥39.0℃,high LYM,RDW and LDH as well as≥2 lobes of lung involved were independent risk factors for MPP children complicated with liver damage(P<0.05).The ROC curve analysis showed that the critical value of LYM≥46.25%,heat duration≥5.50 days,RDW≥12.95%,LDH≥298.00 U/L,involvement of lung lobe≥2 and heat peak≥39.0℃could be used as predictive indexes of MPP complicated with liver damage.The areas under the curve were 0.552,0.700,0.713,0.809,0.634 and 0.703,respectively,and the joint prediction area under the curve was 0.884.Conclusion:When the heat duration≥5.50 days,RDW≥12.95%and LDH≥298.00 U/L occurred in children with MPP,the possibility of liver damage was higher.In clinical practice,pediatricians should pay attention to the above indexes and factors,raise their vigilance to the liver damage from Mycoplasma pneumoniae pneumonia in children,and take timely treatment measures.
