Study on the Effect of Global Budget of Medical Insurance in County-level Medical Community
Objective To evaluate the policy effect of global budget of medical insurance in county-level medical community,and to provide reference for further promoting the linkage reform of medical insurance and county-level medical community.Methods The double difference method was used to evaluate the effect of global budget of medical insurance in county-level medical community from five as-pects of medical expenses,hierarchical diagnosis and treatment,medical quality,medical insurance expenditure and personal burden.Re-sults Compared with the non-pilot medical community areas,the average outpatient expenses and the proportion of out-of-pocket medical expenses in the pilot areas decreased by 15.141 yuan and 8.819 percentage points respectively,while the proportion of outpatient medical insurance fund expenditure and the downward referral rate increased by 12.419 percentage points and 1.539 percentage points respectively.Conclusion The implementation of global budget of medical insurance in county-level medical community has a positive effect on improv-ing patients'awareness of cost control,reducing the economic burden of patients'illnesses,promoting the effective use of medical insur-ance funds,and promoting the development of the hierarchical diagnosis and treatment system.However,there is also the dilemma that the progress of the reform is lagging behind the development of the medical communities.It is necessary to improve the payment method of medical insurance in county-level medical community and establish the supporting policies in accordance with the local conditions.
