This paper systematically reviews the application scenarios of virtual reality(VR)technology in foreign hospitals,community rehabilitation centers and patients'homes,and reveals that VR can effectively integrate short-term professional care in medical institutions and professional self-training in communities or families through information-sharing platforms and remote monitoring technology,thereby ensuring the coherence,integrity and effectiveness of rehabilitation.On this basis,this paper proposes a rehabilitation model for chronic diseases of three-level institutional collaboration based on China's national conditions:constructing a multi-institutional information-sharing platform with VR technology as the core,establishing a follow-up system and multidisciplinary cooperation team,developing a patient-side APP for medical services and health management,and creating a VR application environment,etc.
