Objective:To explore the expression level and clinical application value of serum microRNA-185-5p in patients with cervical cancer.Method:The levels of serum exosomal miR-185-5p in 113 cervical cancer pa-tients(cervical cancer group),101 cervical intraepithelial neoplasia(CIN)patients(CIN group)and 95 healthy fe-male controls(healthy control group)were detected by RT-qPCR.The changes in levels of serum exosomal miR-185-5p were analyzed and the association between serum exosomal miR-185-5p levels and clinicopathological charac-teristics was also investigated.Cox regression model was used to analyze the serum exosomal miR-185-5p level and clinicopathological data.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic effi-cacy of serum exosomal miR-185-5p for cervical cancer.Results:The expression levels of serum exosomal miR-185-5p in cervical cancer group and CIN group were significantly lower than that in healthy control group(P<0.01).Meanwhile,expression levels of serum exosomal miR-185-5p in cervical cancer were significantly lower than that in CIN group(P<0.01).The levels of serum exosomal miR-185-5p in cervical cancer patients with FIGO stage Ⅲand Ⅳ,lymph node metastasis and involvement of the vaginal wall were significantly lower than those in patients with FIGO stage Ⅰ and Ⅱ,without lymph node metastasis and without involvement of the vaginal wall,respectively(P<0.05 or P<0.01).The level of serum exosomal miR-185-5p(HR=0.260,P<0.01)and lymph node metas-tasis(HR=1.572,P<0.05)were independent prognostic factors in patients with cervical cancer.The area under the ROC curve for discrimination between cervical cancer and controls was 0.796,with a sensitivity of 67.26%and a specificity of 81.05%.Conclusion:The level of serum exosomal miR-185-5p may contribute to the clinical diagno-sis and early evaluation of prognosis for cervical cancer.
