摘要
目的:探讨核苷酸结合寡聚化结构域样受体蛋白4(NLRC4)、二肽基肽酶Ⅲ(DPP3)、半胱氨酸天冬氨酸蛋白水解酶-8(Caspase-8)对脓毒症心肌损伤的诊断和预后评估价值.方法:选取本院收治的133例脓毒症患者为研究对象,根据是否合并心肌损分为心肌损伤组(44例)和非心肌损伤组(89例),根据患者预后将心肌损伤患者分为存活组(28例)和死亡组(16例),比较不同组间NLRC4、DPP3、Caspase-8水平;并分析心肌损伤组NLRC4、DPP3、Caspase-8水平与氨基末端脑钠肽前体(NT-proBNP)、心肌肌钙蛋白(cTnI)、肌酸激酶同工酶(CK-MB)水平的相关性.ROC曲线分析NLRC4、DPP3、Caspase-8对心肌损伤的诊断和预后评估价值.结果:与非心肌损伤组比较,心肌损伤组患者NLRC4、DPP3、Caspase-8水平升高,与存活组比较,死亡组NLRC4、DPP3、Caspase-8水平升高(P＜0.05),且 NLRC4、DPP3、Caspase-8 水平与 NT-proBNP、cTnI、CK-MB 水平均呈正相关(P＜0.05);ROC 曲线分析结果显示,NLRC4、DPP3、Caspase-8单独及联合检测诊断心肌损伤的AUC均＞0.8,评估患者预后的AUC均＞0.7.结论:NLRC4、DPP3、Caspase-8与脓毒症心肌损伤密切相关,且三者在心肌损伤的诊断及预后评估中均具有一定临床参考价值.
Abstract
Objective:To investigate the expression levels of nucleotide-bound oligomeric domain-like receptor protein 4(NLRC4),dipeptidyl peptidase Ⅲ(DPP3)and cysteine aspartate proteolytic enzyme-8(Caspase-8)in pa-tients with septic myocardial injury and their clinical value in diagnosis and prognosis assessment.Method:133 pa-tients with sepsis in our hospital were selected and divided into myocardial injury group(44 cases)and non-myocar-dial injury group(89 cases)according to whether they were complicated with myocardial injury.According to the prognosis,the patients with myocardial injury were divided into survival group(28 cases)and death group(16 ca-ses).NLRC4,DPP3 and Caspase-8 levels were compared between the two groups.The correlation between the lev-els of NLRC4,DPP3,Caspase-8 and the amino terminal brain natriuretic peptide precursor(NT-proBNP),cardiac troponin(cTnI)and creatine kinase isoenzyme(CK-MB)in the myocardial injury group was analyzed.ROC curve was used to analyze the clinical value of NLRC4,DPP3 and Caspase-8 in the diagnosis and prognosis of myocardial injury.Results:Compared with non-myocardial injury group,NLRC4,DPP3 and Caspase-8 levels in myocardial in-jury group were higher,and NLRC4,DPP3 and Caspase-8 levels were positively correlated with NT-proBNP,cTnI and CK-MB levels(P＜0.05).ROC curve analysis results showed that NLRC4,DPP3,Caspase-8 alone and com-bined detection in the diagnosis of myocardial injury AUC＞0.8.Compared with the survival group,the levels of NLRC4,DPP3 and Caspase-8 in the death group were higher(P＜0.05),and the AUC of NLRC4,DPP3 and Caspase-8 as assessed by alone or in combination was all greater than 0.7.Conclusion:NLRC4,DPP3 and Caspase-8 are closely related to myocardial injury in sepsis,and the three tests alone or in combination have certain clinical value in the diagnosis of myocardial injury and prognosis assessment.
