Although existing continuous care can provide chronic disease patients with continuous medical service after discharge and improve their health condition and quality of life,it still can not meet the individualized service requirements of different patients.Therefore,it is very important to clarify the needs of continuous care in patients with chronic diseases and carry out continuous nursing services based on their needs.This article reviews the concept,significance,present situation,assessment instrument and influencing factors of the needs of continuous care for patients with chronic diseases,so as to provide a reference for further clinical research and improve the quality of continuous nursing services for chronic diseases in China.
