摘要
目的 系统整合关于社区老年人及其家属参与预立医疗照护计划(ACP)体验的质性研究,为ACP在社区老年人中的应用和发展提供循证依据.方法 运用PICoS模型构建研究问题和文献纳入排除标准.计算机检索PubMed、Web of Science、Cochrane Library、中国知网、万方数据库、维普网和中国生物医学文献数据库,检索社区老年人及其家属参与ACP体验的质性研究,检索时限为建库至2023年10月31日.采用澳大利亚乔安娜布里格斯研究所循证卫生保健中心的质性研究评价工具对纳入文献进行质量评价,并运用汇集性Meta整合方法对结果进行整合.结果 共纳入13篇文献,提取出50个主题,归纳成6个类别,整合得出2个整合结果:实施针对性ACP能带来益处,ACP沟通受自身、家庭、医院、社会四方面影响.结论 有关部门应结合我国文化特点,加强ACP相关知识宣教,通过专业人员团队协作,为老年人及其家属提供多方面支持,以促进符合中国文化特色的ACP相关实践的发展与完善.
Abstract
Objective To systematically integrate qualitative research on the experience of community-dwelling older adults and their family members participating in advance care planning(ACP),so as to provide evidence-based basis for the application and development of ACP among community-dwelling older adults.Methods The participant,interest of phenomenon,context,study design(PICoS)model was used to construct research questions,inclusion and exclusion criteria.The qualitative research on the experience of community-dwelling older adults and their family members participating in ACP was electronically retrieved from PubMed,Web of Science,Cochrane Library,China National Knowledge Infrastructure,WanFang Data,VIP,and China Biology Medicine disc.The search period was from database establishment to October 31,2023.Quality evaluation criteria for qualitative research of the Joanna Briggs Institute Evidence-Based Health Care Center was used to assess the quality of the included literature.The aggregation Meta-synthesis method was used to integrate the results.Results A total of 13 articles were included.Fifty themes were extracted and classified into six categories,resulting in two integrated outcomes,including the benefits of implementing targeted ACP and the impact of oneself,family,hospital,and society on ACP communication.Conclusions Relevant departments should combine the cultural characteristics of our country,strengthen ACP knowledge education,and provide multi-faceted support for older adults and their family members through professional team collaboration,in order to promote the development and improvement of ACP related practices that are in line with Chinese cultural characteristics.
