Effect of PRF on bone and soft tissue healing in extraction sockets of elderly patients with T2DM with chronic periodontal extraction
Objective To investigate the effect of platelet-rich fibrin(PRF)on bone and soft tissue healing in extraction sockets of elderly patients with type 2 diabetes mellitus(T2DM)with chronic periodontal extraction.Methods Ninety-six elderly patients with T2DM who were in the resting stage of chronic and required tooth extraction due to periodontitis and were admitted to our hospital from February 2020to April 2023 were randomly divided into three groups according to the number table method,32 cases in each group,with PRF suture placed in the extraction sockets of the study group,no material placed and sutured in the normal scratch extraction sockets of the control group,and no material placed in the normal scratch extraction sockets of the blank group.No material was placed in the blank group.The bone mineral density of the 3 groups before and 12 weeks after tooth extraction was compared,the soft tissue healing score,gingival sulcus fluid cytokines[platelet-derived growth factor AB(PDGF-AB),transforming growth factor β(TGF-β1),interleukin-1 β(IL-1β),interleukin-4(IL-4),vascular endothelial growth factor(VEGF)]and pain score were compared between the three groups at 30 min,1 week,4 weeks and 12 weeks after surgery.Results The patients in the study group had higher bone density in the extraction sockets at 12 weeks postoperatively than in the control and blank groups,with statistically significant differences(P<0.05).The soft tissue healing scores of the extraction sockets of the patients in the study group were lower than those of the control and blank groups at 1,4 and 12 weeks after surgery,while the control group was lower than the blank group,and the differences were statistically significant(P<0.05).PDGF-AB and TGF-β1 concentrations in the gingival sulcus fluid of patients in the study group were higher than those in the control and blank groups at 30 min and 12 weeks after surgery;The concentrations of PDGF-AB,TGF-β1,IL-1β,and IL-4 in the gingival sulcus fluid of patients in the study group at 1 and 4 weeks postoperatively were higher than those in the control and blank groups,and the concentrations of VEGF were lower than those in the blank groups and control groups,with statistically significant differences(P<0.05).Patients in the study group had lower postoperative pain scores at 1 week and 4 weeks than the blank groups and control groups,with statistically significant differences(P<0.05).Conclusion PRF can promote the healing of bone and soft tissues in extraction sockets,reduce the inflammatory response and decrease the degree of pain in elderly patients with T2DM with chronic,and has anti-inflammatory,analgesic and healing effects.
