目的:评价单侧双通道内镜技术(UBE)联合局部使用糖皮质激素治疗腰椎管狭窄症(LSS)的早期疗效.方法:选取 60 例诊断为单节段LSS且接受UBE手术治疗的患者,随机分为实验组、对照组各 30 例,实验组术中使用糖皮质激素浸润明胶海绵,对照组使用生理盐水浸润明胶海绵.记录手术时间和并发症,并对比两组患者腰腿痛、功能改善情况及预后.结果:两组患者均顺利完成手术,两组手术时间无明显差异(P＞0.05),且无并发症发生.两组术后第 1、3天及末次随访时 VAS 评分和ODI评分均较术前改善,且实验组优于对照组,(P均＜0.05).结论:UBE手术中局部使用糖皮质激素能有效降低术后早期疼痛,加速腰部及下肢功能恢复.
Early efficacy of UBE combined with topical glucocorticoids in lumbar spinal stenosis
Objective:To evaluate the early efficacy of unilateral two-channel endoscopic technique(UBE)combined with topical glucocorticoid in the treatment of lumbar spinal stenosis(LSS).Methods:Sixty patients diagnosed with single-segment LSS and treated with UBE were randomly categorized into 30 cases each in the experimental group and the control group.The experimental group was treated with glucocorticoid-impregnated gelatin sponges during the operation,and the control group was treated with saline-impregnated gelatin sponges.The operation time and complications were recorded,and the low back pain,functional improvement and prognosis of patients in the two groups were compared.Results:The operation was successfully completed in both groups,and there was no apparent difference in operation time between the two groups(P＞0.05),and no complications occurred.The VAS and ODI scores of the two groups improved on the first and third postoperative days and at the last follow-up,and the experimental group was better than the control group(P<0.05).Conclusion:Local use of glucocorticoids in UBE surgery can effectively reduce the short-term postoperative pain and accelerate the functional recovery.
