摘要
分析重症住院中心静脉置管(CVC)患者并发多重耐药菌(MDRO)感染的危险因素,并构建预测模型。选择2019年6月到2022年6月重症住院中心收治的316名CVC患者为研究对象,将其分为感染组(80例)和对照组(236例)。采用Logistic回归分析影响CVC患者并发MDRO感染的危险因素,构建预测模型,分析其实际应用价值。Logistic回归分析提示,年龄>60岁、机械通气、抗生素使用时间>10 d、抗生素种类三联及以上、合并糖尿病以及股静脉置管是引起重症住院CVC置管患者发生MDRO感染的独立危险因素。预测模型的AUC=0.945,灵敏度及特异度分别为91.3%和97.5%。基于以上分析构建的预测模型在筛选高危MDRO感染者中的价值良好。 The purpose of this study was to identify the risk factors of multi-drug resistant organism (MDRO) infections in patients with central venous catheterization (CVC) in a critical care medicine center, and accordingly to construct a prediction model. A total of 316 patients with CVC admitted to our critical care medicine center between June 2019 and June 2022 were included as study subjects and assigned to the infection group (n=80) and the control group (n=236). Logistic regression analysis was performed to identify the risk factors affecting comorbidity with MDRO infection in patients with CVC. A prediction model was thereby constructed and validated for its usefulness. Our Logistic regression analysis showed that patient age > 60 years old, use of mechanical ventilation, duration on antibiotics > 10 days, combination use of three or more antibiotics, diabetes mellitus, and femoral vein catheterization were independent risk factors for MDRO infections in patients with CVC who stayed in our critical care medicine center. The prediction model yielded an area under curve (AUC) of 0.945, a sensitivity of 91.3% and a specificity of 97.5%. In conclusion, the prediction model constructed based on our data analyses is considerably valuable in screening patients at high-risk for MDRO infection.
