Objective To investigate the clinical features of renal perivascular epithelioid cell tumor(PEComa).Methods The clinical data of 19 patients with renal PEComa admitted to the Department of Urology,the First Affiliated Hospital of Soochow University from January 2019 to April 2024 were retrospectively analyzed.The clinical characteristics,imaging findings,pathological features,treatment and prognosis of the patients were analyzed.Results All the 19 patients un-derwent surgical treatment,including 15 partial nephrectomies and 4 radical nephrectomies.Open surgery was performed on 1 case,Robot-assisted laparoscopic surgery was performed on 2 cases and conventional laparoscopic surgery was performed on 16 cases.Pathological diagnosis of the patients were all PEComas.Immunohistochemical staining was positive in HMB-45 84.2％(16/19),Melan-A94.7％(18/19),SMA 100％(18/18),Desmin 83.3％(15/18),S100 53.8％(7/13),CD3461.5％(8/13).Ki-67 index expressed in different degrees(1％-30％).The follow-up time of 19 pa-tients was 1-60 months.Two patients died due to other causes,and the other patients did not have metastasis or recurrence.Conclusions Renal PEComa is rare in clinic,with atypical symptoms and imaging manifestations.Pathology is the gold standard for diagnosis.Surgical resection is the first choice for treatment,and most patients have a good prognosis after surgery.
