摘要
目的 探讨血清可溶性内皮糖蛋白(Endoglin)和PSA联合MRI在前列腺癌(PCa)诊断中的临床价值.方法 选取2019年8月至2023年1月于西安医学院第一附属医院确诊的65例PCa患者(PCa组)和73例良性前列腺增生(BPH)患者(BPH组)为研究对象.比较两组的血清Endoglin、PSA和前列腺MRI各诊断结果的差异,使用受试者特征曲线分析血清Endoglin、PSA和前列腺MRI联合检测的诊断效能.同时,对联合检测的诊断预测模型进行校准度、临床决策曲线和临床影响曲线的验证.结果 血清Endoglin、PSA和MRI分别在PCa诊断中的敏感度和特异性为91.0%、91.7%,90.0%、95.0%和90.0%、97.0%.联合检测的敏感度、特异性和诊断符合率分别为98.5%、97.3%和96.0%.诊断预测模型具有良好的校准度(P=0.151),在联合诊断发生阈值概率大于18%时,临床净获益水平较高.结论 血清Endoglin和PSA联合前列腺MRI诊断在PCa诊断中具有高效能,其诊断预测模型准确可靠,具有重要的临床应用价值.
Abstract
Objective To investigate the clinical value of serum soluble Endoglin and prostate-specific antigen(PSA)combined with magnetic resonance imaging(MRI)in the diagnosis of pros-tate cancer(PCa).Methods A total of 65 patients with PCa(PCa group)and 73 patients with be-nign prostatic hyperplasia(BPH)(BPH group)diagnosed at the First Affiliated Hospital of Xi'an Medical University from August 2019 to January 2023 were selected as study subjects.The differ-ences in serum Endoglin,PSA,and prostate MRI diagnostic results between the two groups were compared.The diagnostic efficacy of combined detection of serum Endoglin,PSA,and prostate MRI was analyzed using receiver operating characteristic curves.Additionally,the diagnostic prediction model for combined detection was validated through calibration,decision curve analysis,and clinical impact curve.Results The sensitivity and specificity of serum Endoglin,PSA,and MRI in the di-agnosis of PCa were 91.0%and 91.7%,90.0%and 95.0%,and 90.0%and 97.0%,respectively.The sensitivity,specificity,and diagnostic accuracy of combined detection were 98.5%,97.3%,and 96.0%,respectively.The diagnostic prediction model demonstrated good calibration(P=0.151),with a high level of clinical net benefit when the threshold probability for combined diagnosis exceeded 18%.Conclusions The combined diagnosis of serum Endoglin PSA and prostate MRI exhibits high efficacy in the diagnosis of PCa,and its diagnostic prediction model is accurate and reli-able,making it an important tool with significant clinical application value.
