摘要
目的 分析唐山地区女性人乳头瘤病毒(HPV)感染阳性率、基因分型及与宫颈病变的相关性.方法 采用PCR以及线性杂交法(LA)对2 557例女性宫颈脱落细胞标本进行HPV检测和基因分型.结果 2 557例患者中HPV阳性675人,阳性率26.4%; 20岁以下阳性率(66.7%)显著高于其余各组(P<0.05),20岁以上各组阳性率在20.9% ~ 27.4%之间,组间差异无统计学意义(P>0.05);高危型以HPV16、58、52基因型为主,低危型以HPVCP8304、6、11基因型为主;HPV16在宫颈癌、宫颈上皮内瘤变(CIN)和慢性宫颈炎的阳性率分别为51.8%、67.6%和32.6%.结论 唐山地区HPV感染的主要基因型与国内其他地区报道基本一致,HPV基因分型对该地区女性HPV感染前的免疫接种具有较好的指导意义.
Abstract
Objective The study was conducted to analyze the correlation between positive rate of human papilloma virus (HPV) infection,genotypes of HPV,and cervical lesions among women in Tangshan.Methods 2 557 specimens of female cervical exfoliated cells were subjected to HPV detection and genotyping by PCR and Linear Array (LA) methods.Results Among the 2557 patients,675 cases were tested HPV-positive,with a positive rate of 26.4%.The positive rate among patients under the age of 20 (66.7%) was significantly higher than that among other groups (P<0.05); the positive rates fell between 20.9%-27.4% among patients above the age of 20,and no statistical difference in positive rates was observed between different age groups (P>0.05).HPV16,58,and 52 were the main high-risk genotypes,while HPVCP8304,6,and 11 were the main low-risk genotypes.The positive rates of HPV16 in cervical cancer,cervical intraepithelial neoplasias (CIN),and chronic cervicitis were 51.8%,67.6%,and 32.6%,respectively.Conclusion The main genotypes of HPV infection in Tangshan is consistent with those reported in other domestic areas,suggesting that HPV genotyping provides good guidelines for the immunization for women before HPV infections.
