ROSE技术联合电子支气管镜对多重耐药感染患者细菌培养阳性率、炎症指标及疗效的影响

目的 探讨ROSE技术联合电子支气管镜对多重耐药感染患者细菌培养阳性率、炎症指标及疗效的影响,为临床实际应用提供参考.方法 选取 2023 年 1 月—2024 年 4 月于新疆生产建设兵团医院呼吸与危重症医学科RICU和ICU住院治疗的最终诊断为多重耐药感染并在住院期间行电子支气管镜检查术的重症监护病房住院患者 80 例.根据是否接受ROSE干预分为ROSE组和未行ROSE组.未行ROSE组在住院期间单纯行电子支气管镜(奥林巴斯,BF-Q290)检查术.ROSE组在住院期间行ROSE技术联合电子支气管镜检查术.对比两组细菌阴转率、定植率、感染率、住院期间总花费、住院时间、抗生素使用时间、临床疗效、炎症等指标水平.结果 治疗后,ROSE组的细菌阴转率高于未行ROSE组,定植率和感染率低于未行ROSE组,差异有统计学意义(P<0.05).ROSE组住院期间总花费、住院时间、抗生素使用时间等指标均低于未行ROSE组,差异有统计学意义(P<0.05).ROSE组总有效为 97.50%,高于未行ROSE组的 75.00%,差异有统计学意义(P<0.05).治疗后,ROSE组CRP、IL-4、IL-6 等指标均低于未行ROSE组,差异有统计学意义(P<0.05).结论 ROSE技术有助于增加行电子支气管镜检查的多重耐药感染患者细菌阴转率,降低定植率和感染率,效果显著,且节省住院期间总花费,缩短住院时间和抗生素使用时间,改善CRP、IL-4、IL-6等指标水平.
Effect of ROSE technique combined with electronic bronchoscopy on positive rate of bacterial culture,inflammatory index and curative effect in patients with multi-drug resistant infection
Objective To investigate the effect of ROSE technology combined with electronic bronchoscopy on the positive rate of bacterial culture,inflammatory index and therapeutic effect of patients with multi-drug resistant infection,and to provide reference for clinical application.Methods From January 2023 to November 2024,a total of 80 patients were hospitalized in the intensive care unit of Xinjiang Production and Construction Corps Hospital with a final diagnosis of multidrug resistant infection and underwent electronic bronchoscopy during hospitalization.Whether the citrus received ROSE intervention was divided into ROSE group and ROSE group.The ROSE group only underwent electronic bronchoscopy(Olympus,BF-Q290)during hospitalization.ROSE group underwent ROSE technique combined with electronic bronchoscopy during hospitalization.The negative conversion rate,colonization rate,infection rate,total hospitalization cost,hospital stay,antibiotic use time,clinical efficacy,inflammation and other indicators of the two groups were compared.Results After treatment,the negative conversion rate of bacteria in ROSE group was higher than that in ROSE group without treatment,and the colonization rate and infection rate were lower than that in ROSE group without treatment,with statistical significance(P<0.05).The indexes of total hospitalization cost,hospitalization days and antibiotic use time in ROSE group were lower than those in ROSE group without treatment,and the difference was statistically significant(P<0.05).The total effective rate of ROSE group was 97.50%,which was higher than that of ROSE group(75.00%),and the difference was statistically significant(P<0.05).After treatment,CRP,IL-4,IL-6 and other indexes in ROSE group were lower than those in ROSE group without treatment,and the difference was statistically significant(P<0.05).Conclusion ROSE technique is helpful to increase bacterial negative conversion rate,reduce colonization rate and infection rate in patients with multi-drug resistant infection who underwent electronic bronchoscopy,and it has significant effects,saves total hospitalization cost,shortens hospitalization days and antibiotic use time,and improves CRP,IL-4,IL-6 and other indicators.
