高血压患者主动脉重构与左心房重构的超声分析

摘要
目的:探讨高血压患者主动脉重构与左心房重构的超声分析.方法:选取2021年3月至2022年2月于中西医结合医院的120例高血压患者,均给予超声心动图对左心房内径、主动脉内径进行测量,依据结果分为主动脉和左心房均正常组(A组)、主动脉重构而左心房正常组(B组)、左心房重构而主动脉正常组(C组)、主动脉重构且左心房重构(D组)四组,通过多因素Logistic回归分析探究高血压患者左心房、主动脉重构的危险因素.结果:C组BMI大于A组(P＜0.05);和D组相比,A、B两组的LVMI、舒张压、收缩压均更低(P＜0.05);和C组比,A、B两组的LVMI相对更低(P＜0.05);和A组比,B组的收缩压更高(P＜0.05);和A组比,C组的舒张压处于更高水平(P＜0.05).D组病程大于A组和B组,B组病程大于A组;D组治疗时间小于A组,B组治疗时间小于A组,C组治疗时间小于A组,差异有统计学意义(P＜0.05).D组治疗时间OR值最大为2.155,B组病程OR值最大为1.161,C组病程OR值最大为1.138,A组治疗时间RO值最大为2.522,通过多因素分析可以发现,年龄是最危险的导致主动脉重构的一个独立因素,而LVMI是最危险的导致左心房重构的一个独立因素.结论:最重要导致主动脉重构的独立因素为年龄,最重要的导致左心房重构的独立因素为LVMI.
Abstract
Objective:To investigate the ultrasonic analysis of aortic remodeling and left atrial remodeling in patients with hypertension.Methods:A total of 120 patients with hypertension from March 2021 to February 2022 in the Hospital of Integrated Chinese and Western Medicine were selected,and echocardiography was given to measure the inner diameter of left atrium and aorta.According to the results,the patients were divided into four groups:group A with normal aorta and left atrium(group A),group B with normal aorta,group C with normal aorta,and group D with reconstructed aorta.Multivariate Logistic regression analysis was used to explore the risk factors of left atrium and aorta remodeling in hypertensive patients.Results:BMI of group C was higher than that of group A(P＜0.05).LVMI,diastolic blood pressure and systolic blood pressure in groups A and B were lower than those in group D(P＜0.05).Compared with group C,LVMI in group A and group B was lower(P＜0.05).Compared with group A,the systolic blood pressure in group B was higher(P＜0.05).Compared with group A,the diastolic blood pressure in group C was higher(P＜0.05).The course of disease in group D was longer than that in group A and group B,and the course of disease in group B was longer than that in group A.The treatment time of group D was less than that of group A,group B was less than that of group A,and group C was less than that of group A,the difference was statistically significant(P＜0.05).The maximum OR value of treatment time in group D was 2.155,the maximum OR value of disease duration in group B was 1.161,the maximum OR value of disease duration in group C was 1.138,and the maximum RO value of treatment time in group A was 2.522.Through multivariate analysis,it was found that age was the most dangerous independent factor leading to aortic remodeling.LVMI is the most dangerous independent factor for left atrial remodeling.Conclusion:The most important independent factor leading to aortic remodeling is age,and the most important independent factor leading to left atrial remodeling is LVMI.
