摘要

目的:探讨同反相位回波顺序选择在肝铁沉积与脂肪肝诊断中的价值.材料与方法:回顾性分析32例肝铁沉积及44例脂肪肝在同反相位回波不同顺序的MRI信号特点及信号差值.结果:肝铁沉积在opp-in序列信号反相位较正相位高,in-opp序列反相位较正相位低;opp-in序列中信号差值(正相位减去反相位)为-18.34±9.61,in-opp序列信号差值为40.56±23.11,二者有明显统计学差异(t值-12.789,P<0.001);in-opp序列中易误为脂肪肝.脂肪肝在opp-in序列信号反相位低于正相位;opp-in序列中信号差值为69.59±65.72,in-opp序列中信号差值为86.93±62.94,二者有明显统计学差异(t值-5.295,P<0.001).in-opp序列同反相位差值更大,加重脂肪肝诊断.结论:肝脏同反相位序列回波顺序选择上,反相位在前,同相位在后,有利于肝铁沉积与脂肪肝的鉴别诊断.

Abstract

Purpose:To explore the diagnostic efficiency of TlWI in-phase and out-phase sequences echo selection order in hepatic iron deposition and fatty liver.Materials and Methods:32 cases of hepatic iron deposition and 44 cases of fatty liver were retrospectively analyzed,and the MRI signal characteristics and differences in different sequences of the in-phase and out-phase echoes were analyzed respectively.Results:The out-phase of iron deposition in opp-in sequence was higher than that in in-phase,and the in-phase in in-opp sequence was lower than that in out-phase.The signal difference(in-phase minus out-phase)of opp-in sequence was-18.34±9.61,and that of in-opp sequence was 40.56±23.11,which showed significant difference(T-value-12.789,P<0.001).in-opp sequence is easily misidentified as fatty liver.The out-phase of fatty liver was lower than the in-phase in the opp-in sequence.The signal difference in the opp-in sequence was 69.59±65.72,and that in the in-opp sequence was 86.93±62.94,showing a significant statistical difference(T-value-5.295,P<0.001).In the in-opp sequence,the out-phase difference was larger,which would aggravate the diagnosis of fatty liver.Conclusion:In the sequence selection of liver echo with in-phase and out-phase sequence,out-phase is the first and in-phase is the last,which is beneficial to the diagnosis of hepatic iron deposition and fatty liver.
