摘要
目的:分析胃肠道间质瘤(GISTs)的CT影像学特征与其病理危险度分级的相关性.方法:收集2019年12月至2023年3月本院收治入院的65例胃肠道间质瘤患者的临床资料,均经手术或穿刺活检病理证实及危险度分级,且均接受CT平扫及增强检查.根据病理结果统计GISTs危险度分级的情况,比较不同病理危险度GISTs患者的CT影像学特征.结果:65例GISTs患者中极低危险11例,低危险22例,中危险11例、高危险21例;不同病理危险度GISTs患者的肿瘤是否出现坏死或囊变、出血、粘膜侧溃疡及病灶大小、形态、边界、强化方式比较,差异有统计学意义(P＜0.05);不同病理危险度GISTs患者的肿瘤是否出现钙化、发病部位、生长方式、强化程度、肿瘤有无供血动脉、性别、年龄比较,差异无统计学意义(P＞0.05).结论:在GISTs的CT影像学特征中,肿瘤是否出现坏死或囊变、出血、粘膜侧溃疡及病灶大小、形态、边界、强化方式与恶性肿瘤风险密切相关,通过对GISTs的恶性风险的评估,对临床治疗及预后起到至关重要的作用.
Abstract
Objective:To analyze the correlation of the CT imaging characteristics of the gastrointestinal stromal tumor(GISTs)and the grade of pathological risk in the gists.Methods:The clinical data of 65 patients with gastrointestinal stromal tumor admitted to our hospital from December 2019 to March 2023 were collected.All patients were pathologically confirmed by surgery or puncture biopsy and their risk grade,and all patients received plain CT scan and enhanced CT examination.Based on the pathological results,the risk grade of the GISTs is analyzed,and the CT imaging characteristics of different risk GISTs are compared.Results:11 of the 65 GISTs are in very low risk,22 in low risk,11 in medium risk,and 21 in high risk.The difference in the tumor in different pathological risk GISTs is statistically significant in the presence of necrosis or cystic degeneration,hemorrhage,mucosal side ulcers,and the size,morphology,boundary,and enhancement mode of the lesions(P＜0.05).The presence of calcification,holes and disease sites in the GISTs,growth pattern,degree of enhancement,whether the tumor has blood supplying arteries,sex and age in different risk gists is not statistically significant(P＞0.05).Conclusion:In the CT imaging characteristics of the GISTs,the presence of necrosis or cystic degeneration,bleeding,mucosal side ulcers,and the size,shape,boundary,and enhancement pattern of the lesions are closely related to the risk of malignant tumor in the GISTs.The assessment of the malignant risk of the gists plays a crucial role in clinical treatment and prognosis of the gists.
