高原红细胞增多症颅脑CT表现与血红蛋白含量的相关性分析

摘要
目的:利用CT检查探讨高原红细胞增多症(high altitude polycythemia,HAPC)患者颅内动脉管径、密度的变化,以及二者与血红蛋白(hemoglobin,Hb)含量的相关性,增加对HAPC患者颅脑CT表现的认识、提高HAPC的诊断能力.方法:选取2021年7月至2022年7月在青海省玉树藏族自治州第三人民医院确诊为HAPC患者及生活在同海拔高度的健康对照者各17例.回顾性分析两组患者颅脑CT表现,分析颅内动脉管径大小、密度高低以及二者与Hb的相关性.结果:HAPC患者大脑前动脉(anterior cerebral artery,ACA)、大脑中动脉(middle cerebral artery,MCA)及大脑后动脉(posterior cerebral artery,PCA)管径、密度均大于健康对照者,具有统计学差异(P<0.05).HAPC患者血红蛋白含量大于健康对照者,差异具有统计学意义(P<0.05).结论:HAPC患者颅脑CT主要表现为颅内动脉管径代偿性增粗,密度显著增加,二者表现与Hb呈正相关.结合患者临床表现、实验室检查及颅脑CT表现能够有效提高诊断准确率,减少误诊.
Abstract
Objective:To investigate the changes of intracranial artery diameter and density in patients with high altitude polycythemia(HAPC)by CT examination,as well as their correlation with hemoglobin(Hb)level,to increase the cognition of Head CT signs and improve the diagnostic capability of HAPC.Methods:17 patients with HAPC diagnosed in the Third People's Hospital of Yushu Tibetan Autonomous Prefecture,Qinghai Province from July 2021 to July 2022 and 17 healthy controls living at the same altitude were selected.The Head CT signs of the two groups were analyzed retrospectively to explore the size and density of intracranial arteries and their correlation with Hb level.Results:The diameters and densities of anterior cerebral artery(ACA),middle cerebral artery(MCA)and posterior cerebral artery(PCA)in HAPC patients were significantly larger and higher than those in healthy controls(P<0.05).The Hb level of HAPC patients were higher than those of healthy controls,and the difference was statistically significant(P<0.05).Conclusion:The main manifestations of Head CT in patients with HAPC are compensatory increase of intracranial artery diameter and significant increase in density.The degree of manifestations is positively correlated with Hb level.The combination of patients'clinical manifestations,laboratory examinations and Head CT signs can effectively improve the diagnostic accuracy and reduce misdiagnosis.
