摘要
目的:探究多参数磁共振成像(multi-parameter magnetic resonance imaging,mp-MRI)在小儿毛细胞型星形细胞瘤的诊断价值.方法:回顾性分析2018年9月至2023年9月医院收治30例疑似毛细胞型星形细胞瘤患儿临床资料.患儿入院后均接受磁共振成像技术(magnetic resonance imaging,MRI)检查,疾病得到病理诊断证实.以病理诊断结果为"金标准"评估常规 MRI(T1WI+T2WI)、双参数 MRI(biparametric magnetic resonance imaging,bp-MRI)(T1WI+T2WI+DWI)、mp-MRI(T1WI+T2WI+DWI+DSC-PWI)对小儿毛细胞型星形细胞瘤的诊断效能,并进行对比分析.结果:病理诊断结果显示30例患儿中24例为小儿毛细胞型星形细胞瘤;常规MRI、bp-MR、mp-MRI对小儿毛细胞型星形细胞瘤诊断的阳性预测值分别为73.68%、90.00%、95.83%,阴性预测值分别为9.09%、40.00%、83.33%,灵敏度分别为 58.33%、75.00%、95.83%,特异度分别为 16.67%、66.67%、83.33%,准确度分别为50.00%、73.33%、93.33%;三组方法灵敏度、准确度差异有统计学意义(P>0.05).结论:小儿毛细胞型星形细胞瘤临床诊断过程中,采用mp-MRI可获取较高诊断效能,可为患儿精准治疗提供可靠影像学检查依据.
Abstract
Objective:To explore the diagnostic value of multiparameter magnetic resonance imaging(multi-parameter magnetic resonance imaging,mp-MRI)in pediatric pilocytic astrocytoma.Methods:Clinical data of 30 children with suspected pilocytic astrocytoma who were admitted to the hospital from September 2018 to September 2023 were also analyzed.After admission,the children underwent magnetic resonance imaging technology(magnetic resonance imaging,MRI)examination,and the disease was confirmed by pathological diagnosis.The diagnostic efficacy of conventional MRI(T1WI+T2WI),two-parameter MRI(biparametric magnetic resonance imaging,bp-MRI)(T1WI+T2WI+DWI),mp-MRI(T1WI+T2WI+DWI+DSC-PWI)in pediatric astrocytomas was evaluated as the"gold standard".Results:The pathological diagnosis results showed that 24 out of 30 children were pediatric pilocytic astrocytoma;The positive predictive value of conventional MRI,bp-MR,and mp-MRI for pediatric pilocytic astrocytoma diagnosis was 73.68%,90.00%,and 95.83%,respectively,The negative predictive values were 9.09%,40.00%,and 83.33%,Sensitivity was 58.33%,75.00%,95.83%,The arities were 16.67%,66.67%,and 83.33%,respectively,The accuracy was 50.00%,73.33%and 93.33%respectively;The difference in sensitivity and accuracy of the three methods was statistically significant(P>0.05).Conclusion:In the clinical diagnosis of pediatric pilcytic astrocytoma,mp-MRI can provide reliable imaging basis for accurate treatment of children.
