摘要

原发性中枢神经系统血管炎是一种罕见病,可引起一系列中枢神经系统异常,是导致颅内动脉狭窄及缺血性卒中的重要原因,因此,早期诊断和治疗对该疾病的预后和结局至关重要.高分辨率磁共振血管壁成像对血管壁和管腔结构的改变具有独特的显示效果,对原发性中枢神经系统血管炎病因学诊断具有重要价值.随着临床实践中对颅内高分辨率磁共振血管壁成像的需求增加,作为放射科医师应了解成像参数对图像质量、临床适应证、评估结果的影响,探究破除图像局限性对原发性中枢神经系统血管炎诊断、预后和指导临床治疗决策的重要意义.本文就高分辨率磁共振血管壁成像对原发性中枢神经系统血管炎的成像方法、影像学表现及诊断价值展开综述.

Abstract

Primary central nervous system vasculitis is a rare disease that causes a spectrum of central nervous system abnormalities and is an important cause of intracranial arterial stenosis and ischemic stroke,therefore,early diagnosis and treatment are essential for the prognosis and outcome of this disease.High-resolution magnetic resonance angiography has a unique display effect on the changes of vascular wall and lumen structure,and is of great value in the etiological diagnosis of primary central nervous system vasculitis.With the increasing demand for intracranial high-resolution magnetic resonance vascular wall imaging in clinical practice,as radiologists should understand the impact of imaging parameters on image quality,clinical indications,and evaluation results,and explore the important significance of eliminating image limitations for the diagnosis,prognosis,and guiding clinical treatment decisions of primary central nervous system vasculitis.This article reviews the imaging methods,imaging findings and diagnostic value of high-resolution magnetic resonance vascular wall imaging in primary central nervous system vasculitis.
