超声诊断下肢静脉血栓的临床价值分析

摘要
目的:分析多普勒超声联合二维超声图像特点对下肢静脉血栓的诊断价值.方法:选取2019年6月至2022年8月我院收治的102例下肢静脉血栓患者作为研究对象,所有患者均通过多普勒超声联合二维超声检查,回顾性分析临床资料和超声表现,并与术后病理学结果进行比较,对比超声诊断结果及其影像特征.结果:与组织病理学诊断结果对比,超声诊断总正确率为93.14％;栓塞部位分别为双侧37例(占36.27％),单右侧31例(占30.39％),单左侧34例(占33.33％).经二维及彩色多普勒显像图多显示为肌间静脉扩张,内见低回声充填;胫后静脉内血栓形成,前、后方胫后静脉管腔内均见低回声充填;探头加压后管腔均不可完全压瘪;胫后静脉管腔内低回声处未见血流信号显示,邻近的胫后动脉有少量血流信号.结论:多普勒超声联合二维超声图像特点针对下肢静脉血栓的诊断正确率较高,能精准识别下肢静脉血栓不同类型的影像特征.应用该种方法可尽早发现与诊断下肢静脉血栓情况,可将其作为相关临床诊断的首选方法.
Abstract
Objective:To analyze the diagnostic value of Doppler ultrasound combined with two-dimensional ultrasound image characteristics in lower extremity venous thrombosis.Methods:March 2019 to June 2022,our hospital 102 cases of lower limb venous thrombosis patients as the research object,all patients by doppler ultrasound in combination with two-dimensional ultrasound examination,clinical data were retrospectively analyzed and sonographic findings,and compared with postoperative pathological results contrast ultrasound diagnosis and its imaging features.Results:Compared with the results of histopathology,the total correct rate of ultrasound diagnosis was 93.14％.The sites of embolism were bilateral in 37 cases(36.27％),single right in 31 cases(30.39％)and single left in 34 cases(33.33％).Two-dimensional and color Doppler imaging showed dilated intermuscular veins with hypoechoic filling.Thrombosis was found in the posterior tibial vein,and hypoechoic filling was observed in the lumen of anterior and posterior posterior tibial vein.The lumen of the probe could not be completely compressed after pressure;There was no blood flow signal in the lumen of the inferior tibial vein and a small amount of blood flow signal in the adjacent posterior tibial artery.Conclusion:Doppler ultrasound combined with two-dimensional ultrasound image features has a high diagnostic accuracy for lower extremity venous thrombosis,which can accurately identify different types of image features of lower extremity venous thrombosis.This method can be used to detect and diagnose lower extremity venous thrombosis as early as possible,and it can be used as the first choice for relevant clinical diagnosis.
