结肠黏膜气压伤1例病例报道及规范诊疗建议

摘要
本文报道了 1例在结肠镜检查中较为罕见的由充气造成的猫抓样结肠,患者经过内镜下创面封闭治疗及入院观察治疗4d后康复.在总结临床经验和既往文献资料的基础上,建议增加结肠黏膜气压伤这一概念以完善猫抓样结肠理论,并提出规范的诊疗分级,同时提出了无明显内镜表现的结肠黏膜气压伤这一临床现象并描述其临床特点.
Abstract
This article reported a rare case of cat scratch colon caused by high airflow of colonoscope.The trauma was treated by endoscopic titanium clip closure.The patient was hospitalized for supportive treatment and recovered within 4 days.Based on previous experience and published literatures,the concept of colonic mucosal barotrauma and standardized grading for diagnosis and treatment were raised to support the cat scratch colon theory.At the same time,we proposed a clinical phenomenon of colonic mucosal barotrauma without obvious endoscopic manifestations and outlined its characteristics.
