摘要
目的 探讨帕金森病(Parkinson's disease,PD)患者外周血中铁代谢与炎症因子、临床症状及病情严重程度的相关性.方法 选择2022年5月至2023年5月在新疆医科大学第一、二附属医院就诊的50例PD患者(PD组)和50例健康体检者(对照组).采用酶联免疫吸附试验(ELISA)检测并比较两组外周血中铁、铁转运蛋白二价金属离子转运体1(DMT1)、膜铁转运蛋白1(Ferroportin 1,FPN1)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)及肿瘤细胞坏死因子-a(TNF-a)水平.采用统一帕金森病量表Ⅲ(UPDRSⅢ)、Hoehn-Yahr(H-Y)分期、汉密顿焦虑量表(HAMA)、汉密顿抑郁量表(H AMD)、简易精神状态评价量表(MMSE)及蒙特利尔认知评估量表(MoCA)对PD患者的临床症状及临床严重程度进行评估.采用t检验和Wilcoxon秩和检验分析铁代谢与PD患者炎症因子、临床症状及临床严重程度的相关性.结果 PD组DMT 1水平明显高于对照组(P<0.001);PD组FPN1水平低于对照组(P<0.05);PD组铁水平中位数明显高于对照组(P<0.001).PD组IL-6、IL-1β及TNF-a炎症因子水平中位数高于对照组(P<0.05).PD 患者 DMT1 水平与 IL-6 和 TNF-α 水平相关(P<0.05);FPN1 水平与 IL-6、IL-1β 及TNF-a无相关性.PD患者DMT1水平与UPDRS Ⅲ评分相关(r=0.454,P=0.001);PD患者IL-1β水平与UPDRS Ⅲ评分相关(r=-0.288,P=0.043);PD患者IL-6水平与MoCA评分相关(r=0.420,P=0.002);PD患者中TNF-a水平与病程严重程度相关(r=0.294,P=0.038).结论 PD患者发病可能与血清中铁代谢蛋白紊乱、炎症因子和PD运动症状存在相关性.
Abstract
Objective To explore the correlation between peripheral iron metabolism and inflammatory factors,clin-ical symptoms and severity in patients with Parkinson's disease(PD).Methods Select 50 PD patients(PD group)and 50 healthy individuals(control group)who received medical examinations at the hospitals from May 2022 to May 2023.Enzyme linked immunosorbent assay(ELISA)was used to detect and compare peripheral blood iron,DMT1,FPN1,IL-1β,IL-6 and TNF-α between the 2 groups.The clinical symptoms(motor symptoms and non-motor symptoms)and clinical severity of PD patients were evaluated with unified PD scale Ⅲ(UPDRSⅢ),Hoe-hn-Yahr(HmurY)staging,Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD),mini-mental state assessment scale(MMSE)and Montreal cognitive assessment scale(MoCA).T-test and Wilcoxon rank sum test were used to analyze the correlation between iron metabolism and inflammatory factors,clinical symptoms and clinical severity in patients with PD.Results The content of serum DMT1 in PD group was significantly higher than that in control group(P<0.001);the content of serum FPN1 in PD group was lower than that in control group(P<0.05);and the median level of serum Fe in PD group was significantly higher than that in control group(P<0.001).The median levels of serum IL-6,IL-1β and TNF-α in the PD group were higher than those in the control group(P<0.05).In PD group,the content of DMT1 was correlated with IL-6 and TNF-α,but there was no correlation between serum ferritin FPN1 content and IL-6,IL-1β and TNF-α.The level of DMT1 in PD group was correlated with UPDRSⅢ score,the level of IL-1β in PD group was correlated with UPDRSⅢ score(r=-0.288,P=0.043);the level of IL-6 in PD group was correlated with cognitive function(MOCA)(r=0.420,P=0.002),and the level of TNF-α in PD group was correlated with the severity of disease course(r=0.294,P=0.038).Conclusion The pathogenesis of PD may be related to the disorder of serum iron metabolic proteins and inflammatory factors and the motor symptoms of PD.
