不同角膜人工晶状体测量仪对屈光手术患者眼前节参数测量值的重复性及一致性研究

摘要
目的 研究不同角膜人工晶状体测量仪对屈光手术患者眼前节参数测量值的重复性及一致性.方法 选取2021年4月-2022年4月运城市中心医院眼科收治的进行屈光手术的近视患者100例(200眼)为研究对象,根据不同检测仪器分为Orbscan、Pentacam、Sirius 3组,采用3种仪器分别检测患者陡峭角膜曲率(Steep keratometry,sKm)、平坦角膜曲率(Flat keratometry,fKM)、中央角膜厚度(Central corneal thickness,CCT)、最薄角膜厚度(Thinnest corneal thick-ness,TCT)及前房深度(Anterior chamber depth,ACD).比较3种角膜人工晶状体测量仪评估患者眼前节参数的重复性(TRT)和变异系数(COV)以及三种仪器眼前节参数测量值之间的一致性.结果 与Sirius组和Orbscan组比较,Pentacam组的sKm和fKm测量值的标准差和重复性均较低(P<0.05).与Pentacam组和Orbscan组比较,Sirius组CCT和TCT测量值的标准差和重复性均较低(P<0.05).Orbscan组ACD测量的标准差和重复性与Sirius组和Pentacam组相当.与Sirius组和Orbscan组比较,Pentacam组sKm和fKm测量值COV较低(P<0.05).与Pentacam 组和 Orbscan 组比较,Sirius 组 CCT 测量值 COV 较低(P<0.05).与 Pentacam 组比较,Sirius组和Orbscan组ACD测量值COV较低(P<0.05).与Sirius组和Pentacam组比较,Orbscan 组明显高估了 sKm,明显低估了 fKm 和 ACD(P<0.001).Sirius 组的 sKm、fKm 和ACD测量值更高,而Pentacam组TCT和CCT测量值更高.Orbscan测量的CCT高于Sirius.结论 Pentacam、Orbscan、Sirius测量眼前节参数的重复性较好,但三者参数测量值不一致,有所差异.
Abstract
Objective Study on repeatability and consistency of different corneal intraocular lens measuring instruments in measuring preoperative parameters of patients undergoing refractive surgery.Methods 100 myopic patients(200 eyes)who underwent refractive surgery in the hospital from April 2021 to April 2022 were selected as research subjects.The patients were divided into 3 groups based on different detection in-struments:Orbscan,Pentacamand Sirius.The three instruments were used to measure the patient's steep keratometry(sKm),flat corneal curvature(fKM),central corneal thickness(CCT),thinnest corneal thickness(TCT)and anterior chamber depth(ACD),respectively.Compare the repeatability(TRT)and coefficient of variation(COV)of 3 corneal intraocular lens measuring instruments in evaluating the param-eters of anterior segment of the patients,as well as the consistency of the average measurement values.Results Compared with the Sirius and Orbscan groups,the standard deviation and repeatability of sKm and fKm measurements in the Pentacam group were lower(P<0.05).Compared with the Pentacam and Orbscan groups,the standard deviation and repeatability of CCT and TCT measurements in the Sirius group were lower(P<0.05).The standard deviation and repeatability of ACD measurements in Orbscan group were comparable to those in Sirius and Pentacam groups.Compared with Sirius and Orbscan groups,Pentacam group had lower COV values in sKm and fKm measurements(P<0.05).Compared with Pentacam and Orbscan groups,Sirius group had a lower COV in CCT measurements(P<0.05).Compared with Pentacam group,the COV of ACD measured bySirius group and Orbscan group was lower(P<0.05).Compared with Sirius and Pentacam groups,Orbscan group significantly overestimated sKm and significantly underestimated fKm and ACD(P<0.001).The Sirius group had higher sKm,fKmand ACD measurements,while the Pentacam group had higher TCT and CCT measurements.The CCT meas-ured by Orbscan was higher than that of Sirius.Conclusion Pentacam,Orbscanand Sirius had good re-peatability in the measurement of prenode parameters,but the measured values were different.
