摘要

目的 探讨安氏Ⅱ2错患者力及与髁突形态位置的相关性.方法 选取2017年1月至2020年12月初次就诊于新疆医科大学第一附属医院口腔正畸科的安氏Ⅱ2错患者50例.采用T-scanⅡ数字化咬合分析系统进行咬合接触记录,拍摄锥形术CT(Cone beam computer tomography,CBCT)并测量髁突位置及形态,对所得数据采用SPSS25.0软件进行统计分析.结果 安氏Ⅱ2错患者总力(Total ocdusal force,TOF)、力不对称指数(Asymmetry index of occlusal force,AOF)、前牙力、后牙力、力中心点最大位移(Maximalmovement of COF,MMCOF)在男、女间差异无统计学意义(P>0.05).前牙区力与关节窝深度、髁突近远中径呈正相关(r>0.00,P<0.05);后牙区力与关节窝深度和髁突近远中径呈负相关(r<0.00,P<0.05).后牙区力与Ⅱ类侧TOF呈正相关(r>0.00,P<0.05).结论 安氏Ⅱ2错患者当前牙力增加时,关节窝深度增加,髁突近远中径增宽;而当后牙力增加时,髁突近远中径减小.

Abstract

Objective Exploringthe correlation between occlusal force and condyle morphology in patients with Angle Class Ⅱ2malocclusion.Methods Select 50 Angle Class Ⅱ2 malocclusion who first visited the hospital from January 2017 to December 2020.Record occlusal contact using the T-scanⅡ digital occlusal analysis system,take cone beam computer tomography(CBCT)and measure the position and condyle mor-phology of temporomandibular joint.The obtained data was statistically analyzed using SPSS 25.0.Re-sults There was no statistically significant difference in total occlusal force(TOF),asymmetry index of occlusal force(AOF),anterior and posterior occlusal forcesand maximum movement of center of occlusal force(MMCOF)between males and females in Angle Class Ⅱ,malocclusion patients(P>0.05).The oc-clusal force in the anterior tooth areawas positively correlated with the depth of the joint socket and the mesiodistal diameter of the condyle(r>0.00,P<0.05).The occlusal force in the posterior tooth area was negatively correlated with the depth of the joint socket and the mesiodistal diameter of the condyle(r<0.00,P<0.05).The occlusal force in the posterior tooth area was positively correlated with Class Ⅱ lateral TOF(r>0.00,P<0.05).Conclusion The depth ofanterior fossaincreased in patients with An-gle Class Ⅱ2 malocclusion,the depth of the joint socket increased and the proximal and distal diameters of the condyles widen.When the occlusal force of the posterior teeth increased,the proximal and distal diam-eters of the condyles decreased.
