基于SBAR模式的预检分诊在交通事故致骨折患者中的应用效果

摘要
目的 探究基于现况-背景-评估-建议(Situation background assessment recom-mendation,SBAR)模式的预检分诊在交通事故致骨折患者急诊急救中的应用效果.方法 选取2020年1月-2021年1月就诊于北京积水潭医院急诊科的99例交通事故致骨折患者,根据入院就诊顺序不同分组,对照组采取常规急诊预检分诊(n=49),观察组采取基于SBAR模式的预检分诊(n=50).比较两组诊治情况、预后、并发症、不良事件发生率及就诊服务满意度.结果 观察组分诊评估、分诊挂号、急诊留滞及手术等待时间均短于对照组(P＜0.05);观察组住院、骨折愈合时间均短于对照组(P＜0.05),两组并发症发生率比较,差异无统计学意义(P＞0.05);观察组不良事件发生率低于对照组(P＜0.05);观察组就诊服务满意度量表中候诊时间、候诊环境、就诊秩序、预检服务及信息沟通各项评分均高于对照组(P＜0.05).结论 基于SBAR模式的预检分诊应用于交通事故致骨折患者急诊急救工作中,能有效提高分诊效率、降低不良事件发生率、加速康复进程、提升患者对就诊服务满意度.
Abstract
Objective To investigate the effectiveness of pre-screening triage based on situation background assessment recommendation(SBAR)model in the emergency care of the patients with fractures caused by traffic accidents.Methods 99 patients with traffic fractures who attended the hospital from January 2020 to January 2021 were selected and grouped according to the order of admission.49 patients in the control group were treated by conventional emergency pre-screening,and 50 patients in the observation group were treated by SBAR-based pre-screening.The 2 groups were compared in terms of diagnosis and treat-ment,prognosis and complications,incidence of adverse events and satisfaction with the consultation serv-ice.Results The observation group had shorter waiting time for consultation assessment,triage registra-tion,emergency detention and surgery than the control group(P＜0.05);the observation group had shorter hospitalization and fracture healing time than control group(P＜0.05),and the complication rate was not significantly different from that in control group(P＞0.05);the incidence of adverse events in the observation group lower than that of control group(P＜0.05);the scores of waiting time,waiting envi-ronment,consultation order,pre-screening service and information communication in the consultation service satisfaction scale of observation group were higher than those of control group(P＜0.05).Conclu-sion The application of SBAR-based pre-screening triage to the emergency care of the patients with frac-tures caused by traffic accidents can effectively improve triage efficiency,reduce the incidence of adverse events,speed up the recovery process and increase patient satisfaction with the service.
