Objective To investigate the correlation and predictive value of PIK/Akt/mTOR signaling path-way with clinical features of lung adenocarcinoma in different EGFR mutation states.Methods The sec-ond-generation gene sequencing(NGS)was used to detect 342 patients with lung adenocarcinoma diag-nosed in the hospital from 2010 to 2019.The total amount of DNA should be no less than 150 ng,and the degree of DNA degradation should be≥C.According to whether there was EGFR mutation,the patients were divided into positive group and negative group,and the distribution differences between the 2 groups in different clinicopathological characteristics of lung adenocarcinoma patients were compared.According to the distribution of PIK/Akt/mTOR signaling pathway in 342 patients with lung adenocarcinoma,the correlation between PIK/Akt/mTOR signaling pathway and different mutation states and lymph node me-tastasis of EGFR,as well as the distribution difference of PIK signaling pathway in different mutation states of EGFR and its correlation with T790M mutation state were analyzed.Multivariate Logistic regres-sion was used to analyze the factors affecting EGFR gene mutation.Results(1)The patients with lung adenocarcinoma under 60 years old were more prone to EGFR mutation;(2)The mutation frequency of EGFR in patients with positive expression of phosphatidylinositol-4,5-bisphosphate 3-kinase subunit α(PIK3CA)and phosphatidylinositol 3-kinase(PIK3CG)was higher;(3)The patients with PIK3CA mu-tation were more likely to induce T790M drug-resistant mutation;(4)Lung adenocarcinoma patients with positive expression of PIK3CB and mTOR were more likely to have lymph node metastasis.(5)Age,PIK3CA and PIK3CG expression levels were risk factors for predicting EGFR mutation(P<0.05).Con-clusion EGFR mutation in lung adenocarcinoma was more common in patients under 60 years old.The expression of PIK3CA and PIK3CG was related to EGFR gene mutation.The positive expression of PIK3CA may be involved in regulating the occurrence of T790M drug-resistant mutation.The positive ex-pression of PIK3CB and mTOR will increase the risk of lymph node metastasis in lung adenocarcinoma pa-tients.Age,PIK3CA and PIK3CG were risk factors for predicting EGFR mutation.
