Changes of neutrophil/lymphocyte ratio(NLR)and(hydrogen sulfide)H2S levels in peripheral blood and their correlation with brain injury in children with febrile seizure
Objective To investigate the changes of neutrophil/lymphocyte ratio(NLR)and hydrogen sul-fide(H2S)levels in peripheral blood of children with febrile seizure and their correlation with brain injury.Methods A total of 220 children with febrile seizure admitted to the hospital from January 2021 to January 2023 were selected,of which 153 were in the simple group and 67 were in the complex group.In the same period,109 children with high fever without convulsion were included in the control group.Peripheral blood levels of NLR and H2S were detected in all children,and receiver operating characteristic(ROC)curves were used to distinguish simple and complex febrile seizure.Children with febrile seizure were di-vided into brain injury group and no brain injury group according to the incidence of febrile seizure.Clinical data of the 2 groups were collected.multivariate Logistic regression model was used to analyze the inde-pendent risk factors of febrile seizure brain injury in children.Results Compared with the control group,the level of NLR in peripheral blood of simple and complex group was increased,and the level of H2S was decreased(P<0.05).Compared with the simple group,the level of NLR in peripheral blood of complex group was increased,and the level of H2S was decreased(P<0.05).ROC analysis results showed that the AUC values(95%CI)of NLR and H2S alone and combined detection in peripheral blood were 0.615(0.547~0.680),0.731(0.667~0.788)and 0.827(0.771~0.875)in differentiating children with simple and complex febrile convulsions,respectively.The prediction efficiency of NLR and H2S combined detec-tion was higher than that of each single detection(Z=4.488,2.161,P<0.05).Body temperature≥40℃,complex febrile seizure,number of convulsions ≥2 times,duration ≥15 min,NLR and level of peripheral blood in brain injury group were higher than those in non-brain injury group,and H2S level was lower than that in non-brain injury group(P<0.05).Multivariate Logistic regression model analysis showed that complex febrile seizure,convulsive attacks ≥2 times,convulsive duration ≥15 min,high lev-el of NLR and low level of H2S in peripheral blood were risk factors for brain injury in children with febrile seizure(P<0.05).Conclusion The level of NLR and H2S in peripheral blood of children with febrile sei-zure is increased and the level of H2S is decreased.The level of NLR and H2S in peripheral blood is of great value in differentiating febrile seizure typing in children,and it is closely related to the occurrence of febrile seizure brain injury in children.
