Clinical study of percutaneous transforaminal endoscopic discectomy(PTED)combined with annulus fibrosus repair in treatment of lumbar disc herniation(LDH)
Objective To analyze the clinical efficacy of percutaneous transforaminal endoscopic discectomy(PTED)combined with annulus fibrosus repair in the treatment of lumbar disc herniation(LDH).Methods A total of 117 LDH patients treated with PTED in Shanxi Huajin Orthopedics Hospital from July 2020 to February 2023 were retrospectively analyzed.The patients were divided into 2 groups according to whether annulus fibrosum repair was performed after PTED.The control group(59 cases)was treated with PTED alone,and the observation group(58 cases)was treated with PTED combined with annulus fibrosus su-ture repair.Perioperative indexes,improvement of lumbar pain,lumbar function,complications and post-operative recovery were compared between the 2 groups.Results The operation time of observation group was longer than that of control group(P<0.05).At 1 week and 3 months after theoperation,the visual analogue scale(VAS)of waist and leg in the 2 groups gradually were decreased when compared with those before the operation,and the differences were statistically significant(P<0.05).At 6 and 12 months af-ter the operation,the Japanese Orthopaedic Association(JOA)score of the 2 groups gradually was in-creased when compared with that before the operation,and Qswestry disability index(ODI)score gradual-ly was decreased when compared with that before theoperation,and the differences were statistically sig-nificant(P<0.05).After the operation,the area of responsible spinal canal and lumbar lordosis angle of the 2 groups were significantly higher than those before the operation(P<0.05).After the operation,compared with observation group,the height of responsible intervertebral space in control group was de-creased,and the difference was statistically significant(P<0.05).The recurrence rate of observation group was lower than that of control group,and the difference was statistically significant(P<0.05).Conclusion PTED and PTED combined with annulus suture repair in the clinical treatment of LDH have the effect of promoting postoperative lumbar function recovery and pain relief,and PTED combined with annulus suture repair can maintain the height of intervertebral space and reduce the postoperative recur-rence rate.
