Application of hospital community integrated management in chronic renal insufficiency hemodialysis
Objective To analyze the effect of hospital community integrated management on self-care ability of maintenance he-modialysis patients with chronic renal insufficiency. Methods 116 patients with chronic renal function who underwent hemodiafiltra-tion treatment in Zhumadian Central Hospital from June 2020 to October 2022 were selected and randomized. The control group(58 cases)received routine dialysis care during dialysis, and the study group(58 cases)received the integrated management of the hospi-tal.The serological indicators, nutritional indicators,self-management competence level, complications and adverse events were com-pared between the two groups. Results After intervention, serum β2-microglobulin(β2-MG),urea nitrogen(BUN),parathyroid hor-mone(PTH),blood creatinine(Scr)in the study group were lower than those in the control group, and serum prealbumin(PA),hemo-globin(Hb),(Alb),and(Alb)were higher than those in the control group. The scores of self-care,problem solving, partnership and emotional processing on the self-management ability scale(ESCA)in the study group were higher than those in the control group. There were fewer complications and adverse events during treatment in the study group than those in the control group(P<0.05).Conclusion Integrated community management in hospital can enhance the self-care ability of chronic renal insufficiency, promote the discharge of endotoxin, and improve the nutritional status of the body, with fewer complications and adverse events.
