A Retrospective Analysis of The Use of Prescription Pre-review Intelligent Decision System to Intercept Medication Errors in Emergency Patients
Objective The characteristics of pre-audit intelligent decision system to intercept prescriptions for serious medica-tion errors in emergency patients were analyzed,and suggestions were provided for improving medication safety and rational drug use.Method Extract medication error prescriptions intercepted by the First Affiliated Hospital of Xinjiang Medical University from April 2022 to March 2023 through this system.Retrospective analysis of the characteristics of medication errors in prescriptions.The unquali-fied rate of prescription was compared between manual audit mode and pre-review intelligent decision system.Result The pre-exam-ination intelligent decision system intercepted 5347 wrong prescriptions,accounting for 3.11%of the total prescriptions.Pareto diagram was used to analyze the classification of medication errors.The main factors were dose,inappropriate solvent,route of administration and frequency of administration,accounting for 26.86%,22.69%,12.83%and 10.87%,respectively.Drug use after dilution was the secondary factor,accounting for 10.21%;The general factors were patient conditions,the need for separate prescription,prohibition and improper dosage form selection,accounting for 9.95%,4.10%,1.98%and 0.52%,respectively.The unqualified rate of manual prescrip-tion review was 0.242%,and pre-review intelligent decision system was 0.037%,the difference was statistically significant(P = 0.000).Conclusion The intelligent decision-making system of prescription pre-review can verify the prescription,improve the accuracy of the prescription,and improve the safety of emergency medication under the supervision of the prescription pharmacist.
