摘要
目的 系统评价老年心力衰竭患者衰弱现状及其影响因素.方法 计算机检索从建库-2022年10月PubMed、EmBase、The Cochrane Library、Web of Science、中国知网、万方、维普、中国生物医学文献数据库,收集有关老年心力衰竭患者衰弱发生影响因素的研究,采用Stata16.0进行meta分析.结果 纳入15篇文献,共2856例心力衰竭患者,meta分析结果显示,老年心力衰竭患者衰弱患病率为43％(95％CI:0.35-0.51);影响心衰患者衰弱的各因素OR值及(95％CI)分别为NYHA心功能分级4.11(2.92-5.80)、多重用药 2.72(1.61-4.59)、Charlson合并症指数 1.25(1.08-1.46)、住院时间 0.81(0.74-0.89)、近一年跌倒史0.16(0.07-0.36).结论 老年心衰患者衰弱患病率较高,影响因素较多,有必要开展护理干预改善老年心衰患者衰弱现状.
Abstract
Objective To systematically evaluate the frailty status of elderly patients with heart failure and its influencing factors.Methods Literature retrieved from PubMed,EmBase,The Cochrane Library,Web of Science,China National Knowledge Infrastruc-ture(CNKI),Wan fang,VIP,and Chinese Biomedical Literature Database(CBM)from the establishment to July 2022 were collected to analyze influencing factors of frailty in elderly patients with heart failure,and then meta-analysis was performed using Stata16.0.Re-sults Fifteen papers with 2856 heart failure patients were included,and the results of meta-analysis showed that the prevalence of frailty in elderly patients with heart failure was 43％(95％CI:0.35-0.51,P＜0.01).The ORs and 95％CI of the influencing factors were NYHA cardiac function grade 4.11(2.92-5.80),multidrug grade 2.72(1.61-4.59),Charlson comorbidity index 1.25(1.08-1.46),length of hospital stay 0.81(0.74-0.89),fall history in the last year 0.16(0.07-0.36).Conclusion The prevalence of frailty in elderly patients with heart failure is relatively high,and there are many influencing factors,so it is necessary to carry out nursing in-terventions to improve the current status of frailty in elderly patients with heart failure.
