微导管首过效应在颅内大动脉粥样硬化动脉闭塞的诊断价值分析

目的 研究微导管首过效应在颅内动脉粥样硬化狭窄(intracranial atherosclerotic stenosis,ICAS)大血管闭塞的特异性及敏感性.方法 采用病例-对照研究,选择2021年07月~2022年12月的急性大血管闭塞脑梗死患者60例,分为ICAS组及脑栓塞组.在急诊神经介入治疗前进行微导管首过效应判定,评估微导管首过效应在ICAS大血管闭塞的特异性及敏感性.结果 微导管首通效应检测ICAS大血管闭塞的灵敏度为81.3%,特异性为82.1%,阳性预测值为83.9%,阴性预测值为79.3%,准确率为81.7%.结论 微导管首过效应可作为可靠征象提示ICAS大血管闭塞.
The value of first-pass effect of microcatheter in the diagnosis of intracranial atherosclerotic artery occlusion
Objective Study on the specificity and sensitivity of microcatheter first-pass effect in large vessel occlusion with intra-cranial atherosclerotic stenosis.Methods A case-control study was conducted to select 60 patients with acute large vessel occlusion stroke from July 2021 to December 2022.They were divided into ICAS group and cerebral embolism group.The microcatheter first-pass effect was determined before emergency neurointerventional treatment,and the specificity and sensitivity of microcatheter first-pass ef-fect in ICAS large vessel occlusion were evaluated.Results The sensitivity of microcatheter first-pass effect for detecting ICAS large vessel occlusion was 81.3%,specificity was 82.1%,positive predictive value was 83.9%,negative predictive value was 79.3%,and accu-racy was 81.7%.Conclusion Microcatheter first-pass effect can be used as a reliable sign to indicate ICAS large vessel occlusion.
