Effects of continuous subcutaneous injection with insulin pump on oxidative stress in treatment of DKA

目的 对比胰岛素泵持续皮下注射与胰岛素多次皮下注射治疗糖尿病酮症酸中毒(DKA)的临床疗效及对氧化应激的影响.方法 收集新安中心医院收治的DKA患者75例,按照入院单双号顺序分组,单号患者入观察组(38例),双号入对照组(37例).两组均给予常规基础治疗,在此基础上,观察组采用胰岛素泵持续皮下注射,对照组采用胰岛素每日多次皮下注射.对比两组临床指标、血糖指标、氧化应激水平以及安全性.结果 观察组血糖达标时间、血酮体转阴时间、尿酮体转阴时间以及血pH恢复时间均显著短于对照组,胰岛素用量更低(P<0.05).治疗后两组空腹血糖(FPG)、餐后2h血糖(2 h PG)、糖化血红蛋白(HblAc),超氧化物歧化酶(SOD)、丙二醛(MDA)以及谷胱甘肽过氧化物酶(GSH-Px)均较明显改善,且观察组改善更显著(P<0.05).观察组低血糖发生率明显低于对照组(0 vs 13.51%,P< 0.05).结论 胰岛素泵持续皮下注射对比胰岛素多次皮下注射治疗DKA疗效及安全性更佳,氧化应激状态改善更明显,值得临床推广.
Objective To compare the effects of continuous subcutaneous injection with insulin pump and multiple subcutaneous insulin injections on oxidative stress in the treatment of diabetic ketoacidosis (DKA).Methods A total of 75 patients with DKA admitted to our hospital were selected and divided into an observation group (odd ones) (n=38) and a control group (even ones) (n=37) according to the odd and even serial number of admission.The two groups received routine basic treatment.On this basis,the observation group was further treated with continuous subcutaneous injection with insulin pump and the control group with multiple subcutaneous insulin injections every day.The clinical indicators,glucose levels,oxidative stress levels and safety between the two groups were compared.Results The time for glucose to reach standard level,the time for blood ketone body to turn negative and blood pH recovery time in the observation group were significantly shorter than those in the control group,and the insulin dosage was also less (P < 0.05).After the treatment,the FPG,2h PG,HblAc,SOD,MDA and GSH-Px in the two groups were greatly improved,but the improvement in the observation group was more significant (P < 0.05).The incidence of hypoglycemia in the observation group was significantly lower than that in the control group (0 vs 13.51%,P < 0.05).Conclusion Continuous subcutaneous injection with insulin pump has better effects and safety on the treatment of DKA and can better improve oxidative stress than and multiple subcutaneous insulin injections,and it is worthy of clinical popularization.
