M2型巨噬细胞在哮喘患儿中的表达及益生菌辅助治疗儿童哮喘的研究

目的 探讨M2型巨噬细胞在哮喘患儿中的表达及益生菌辅助治疗儿童哮喘的意义.方法 选取2023年1-6月徐州医科大学附属医院收治的90例支气管哮喘患儿作为哮喘组,另选取同期在本院体检的80例健康儿童作为对照组,比较2组血清精氨酸酶1(Arg-1)、趋化因子22(CCL22)的表达水平.将哮喘患儿按随机数字表法分为A组(吸入用布地奈德混悬液+双歧杆菌四联活菌片)和B组(吸入用布地奈德混悬液),2组均治疗3个月,比较治疗前及治疗3个月后2组患儿的血清白细胞介素-4(IL-4)、白细胞介素-13(IL-13)、Arg-1、CCL22水平的变化,以及肺功能指标的变化.记录治疗前、治疗后第1、3个月哮喘急性发作次数,以评估治疗效果和稳定性.结果 哮喘组Arg-1、CCL22水平高于对照组,差异具有统计学意义(P<0.05).治疗3个月后,A、B组患儿血清IL-4、IL-13、Arg-1、CCL22水平均较治疗前降低(P<0.05);第1秒用力呼气容积占预计值百分比(FEV1%pred)、呼气峰流速占预计值百分比(PEF%pred)、最大呼气中期流量占预计值百分比(MMEF75/25%pred)较治疗前升高(P<0.05),且A组变化程度大于B组,差异有统计学意义(P<0.05).治疗第3个月时,2组患儿的哮喘急性发作次数均较治疗前减少,且治疗第3个月时A组患儿的哮喘急性发作次数显著低于B组,差异有统计学意义(P<0.05).结论 哮喘患儿存在M2型巨噬细胞过度极化及Arg-1、CCL22高表达,这可能在儿童哮喘气道炎症及气道重塑的发生发展中起到一定作用.益生菌可能在一定程度上抑制M2型巨噬细胞过度极化,从而改善哮喘患儿的肺功能及临床症状.
Expression of M2 macrophages in children with asthma and study of probiotic-assisted treatment to pediatric asthma
Objective To investigate the expression of M2 macrophages in children with asthma and the significance of probiotics in the adjuvant treatment of children with asthma.Methods A total of 90 children with bronchial asthma who were admitted to the Affiliated Hospital of Xuzhou Medical University from January 2023 to June 2023 were selected as an asthma group,and 80 healthy children who underwent physical examination at the same hospital during the same period were selected as a control group.The expression levels of serum arginase-1(Arg-1)and chemokine(C-C motif)ligand 22(CCL22)were compared between the two groups.The children with asthma were divided into group A(treated with inhaled budesonide suspension+bifidobacterium)and group B(treated with inhaled budesonide suspension).Both groups were treated for 3 months.The levels of serum interleukin-4(IL-4),interleukin-13(IL-13),Arg-1,and CCL22,as well as pulmonary function parameters were compared before and three months after treatment.The number of acute asthma attacks was recorded before treatment and 1 and 3 months after treatment to evaluate the theraputic effectiveness and stability.Results The levels of Arg-1 and CCL22 in the asthma group were higher than those in the control group,the difference was statistically significant(P<0.05).Three months after treatment,the levels of serum IL-4,IL-13,Arg-1 and CCL22 in groups A and B significantly decreased compared with those before treatment(P<0.05).Percentage of predicted forced expiratory volume in one second(FEV1%pred),percentage of predicted peak expiratory flow(PEF%pred),and percentage of predicted maximum mid-expiratory(MMEF75/25%pred)increased compared with those before treatment(P<0.05),with more significant changes observed in Group A(P<0.05).At the third month of treatment,the number of acute attacks of asthma in both groups decreased compared to before treatment,with group A showing a significantly lower number of acute asthma attacks than Group B at the same time point(P<0.05).Conclusions M2 macrophages are hyperpolarized and Arg-1 and CCL22 are highly expressed in children with asthma,which may play a role in the development and progression of airway inflammation and remodeling in children with asthma.Probiotics may inhibit the excessive polarization of M2 macrophages to a certain extent,thereby improving pulmonary function and clinical symptoms in children with asthma.
