摘要

[目的]建立超高效液相色谱法(UPLC)测定人血清中万古霉素的浓度,为优化临床用药方案提供依据.[方法]选择经HCLO4∶ACN=6∶1 沉淀蛋白及NaOH液萃取进行血浆样本前处理,以去甲万古霉素为内标,处理色谱柱为Shim-pack GIST-HP C18(150 mm×2.1 mm,3 μm),流动相为甲醇-0.05 mol/L的磷酸二氢钾缓冲溶液,流动速度为 0.4 mL/min,柱温40℃,检测波长236 nm,进样量5 μL,对延边大学附属医院使用过万古霉素的患者血药浓度进行监测.[结果]万古霉素在2.5~80 μg/mL质量浓度范围内线性关系良好,回归方程为Y=0.050 4X+0.001 9(r2=0.999 9),定量下限为 2.5 μg/mL.准确度、日间、日内精密度、基质效应、方法回收率和稳定性均符合要求.采用该方法对患者进行血药浓度监测,血药浓度达标率显著升高.[结论]所建立的UPLC测定万古霉素血药浓度操作简便,快速,专属性好,准确度高,重现性好,稳定性强,适用于临床测定万古霉素血药浓度,可为患者制定个体化方案提供参考.

Abstract

OBJECTIVE To establish an ultra performance liquid chromatography(UPLC)method for the determination of vancomycin concentration in human serum,and to provide a basis for optimizing the clinical medication plan.METHODS Plasma samples were pretreated by HCLO4∶ACN=6∶1 precipitated protein and NaOH liquid extraction.Norvancomycin was used as the internal standard.The chromatographic column was Shim-pack GIST-HP C18(150 mm×2.1 mm,3 μm).The mobile phase was methanol-0.05 mol/L potassium dihydrogen phosphate buffer solution,the flow rate was 0.4 mL/min,and the column temperature was 40℃.The detection wavelength was 236 nm,and the injection volume was 5 μL.The blood concentration of vancomycin was monitored in patients treated with vancomycin in the Affiliated Hospital of Yanbian University.RESULTS Vancomycin had a good linear relationship within the mass concentration range of 2.5-80 μg/mL,the regression equation was Y=0.050 4X+0.001 9(r2=0.999 9),and the lower limit of quantitation was 2.5 μg/mL.The accuracy,intra-day and intra-day precision,matrix effect,recovery and stability of the method met the requirements.Using this method to monitor the blood drug concentration of patients,the rate of reaching the standard of blood drug concentration was significantly increased.CONCLUSION The established UPLC method for the determination of vancomycin blood concentration is simple,rapid,specific,high accuracy,good reproducibility and strong stability,and it is suitable for the clinical determination of vancomycin blood concentration and can provide a reference for patients with individualized plans.
