探讨缺血修饰白蛋白、超敏C反应蛋白在微血管性心绞痛诊断中的作用

目的::探讨缺血修饰白蛋白(ischemia modifided albumin,IMA)及超敏C反应蛋白(high sensitive C- reactive protein , hs- CRP)对微血管性心绞痛(Microvascular Angina ,MVA)的诊断价值。方法:选择反复胸痛并行冠状动脉造影提示正常冠状动脉的患者共60例,经心电图运动平板试验分为微血管性心绞痛组(36例)和非心源性胸痛组(24例);并选择同期医院体检的年龄相匹配的健康对照30例。分别测定各组血清IMA、hs- CRP、CK- MB、cTnI。结果:微血管性心绞痛组中血清IMA值高于非心源性胸痛组及正常对照组,差异有统计学意义(P<0.05),在3组比较中,微血管性心绞痛组与非心源性胸痛血清中HS- CRP均较正常对照组增高,但组间比较差异无统计学意义;同时检测了3组CK-MB、cTnI水平组间差异无统计学意义。结论:IMA是心肌缺血较为敏感的生化指标,可以用于微血管性心绞痛和非心源性胸痛的鉴别诊断。
Objective To investigate the diagnosis walue of ischemia modifided albumin (IMA) and onMicrovascular Angina (MVA).Methods Sixty patients of normal coronary by diagnostic coronary angiography for recurrent angina undergo the treadmill test electrocardiogram were divided into two groups:MVA group(36),non- cardiac group(24) ,according to the result of Treadmill exercise test . And thirty normal control with age matched. The serum IMA ,creatine kinase- MB (CK- MB),and troponin I (cTnI)levels were measured .Results The serum IMA levels of MVA group more significantly elevated than the other two group (P<0.05),the serum hs- CRP、CK- MB、cTnI level have no significantly difference between the three group though the hs- CRPlevel higher than the other two group. Conclusion IMA can reflect the early MVA and may be useful as a discriminative marker to distinguish MVA .
