Meta-analysis of the efficacy and safety of high-dose amoxicillin combined with PPI dual therapy in comparison with bismuth quadruple therapy in the initial treatment of Helicobacter pylori infection
Objective To investigate the efficacy and safety of high-dose amoxicillin dual therapy(HDDT)in comparison with bismuth quadruple therapy(BQT)in eradicating Helicobacter pylori(Hp)infection.Methods This study retrieved randomized controlled studies(RCTs)on comparing amoxicillin-containing HD-DT with bismuth-containing BQT in the initial treatment of Hp infection published from January 2017 to June 2023 from databases such as PubMed,EMBase,Web of Science,The Cochrane Library,CNKI,and Wanfang Data Knowledge Service Platform.After strict literature screening,data extraction,and quality evaluation,Meta-analysis was performed using Revman 5.3 software.Results A total of 10 RCTs involving 3 506 pa-tients were included.The Meta-analysis results showed no statistically significant differences(P>0.05)in the incidence rates of constipation,abdominal pain,diarrhea,abdominal distension,dry mouth,oral odor,yellow urine,vomiting,itching,rash,fatigue,insomnia,abnormal sensation/pain in limbs,dizziness,and palpita-tions and intention-to-treat(ITT)and per-protocol(PP)eradication rates between the two groups.The inci-dence rates of belching,nausea,black stools,bitter taste,black tongue/tooth discoloration,decreased appe-tite,headache,and taste disturbances,as well as the overall adverse reaction rate in the HDDT group were lower than those in the BQT group(P<0.05).Conclusion In the eradication of Hp,HDDT containing amoxicillin has similar efficacy to BQT but with a lower rate of adverse reactions.
