目的 探讨联合检测关节液中透明质酸(HA)和炎性因子变化对骨关节炎(OA)的诊断价值.方法 选取2022年1月至2024年1月于江阴市中医院就诊收治的102例OA患者作为观察组,另取同期在该院进行体检的102例健康人群作为对照组.收集所有受试者的关节液并采用ELISA检测关节液中HA和各炎性因子(TNF-α、IL-6、IL-1β)的水平.比较两组间HA、TNF-α、IL-6和IL-1β的变化并评估联合检测关节液中透明质酸和炎性因子变化对OA的诊断价值.结果 ELISA检测结果显示,与对照组比较,观察组中的透明质酸明显下降,TNF-α、IL-6、IL-1β等炎性因子均不同程度升高,两组间差异有统计学意义(P<0.01).多因素逐步Logistic回归分析结果显示:HA(OR=0.912,95%CI:0.858~0.971)是 OA 的保护因素(P<0.01);TNF-α(OR=1.139,95%CI:1.060~1.224)、IL-6(OR=1.110,95%CI:1.069~1.154)和 IL-1β(OR=1.118,95%CI:1.074~1.165)是 OA 的危险因素(P<0.01).ROC 曲线分析结果显示,检测单项的HA、TNF-α、IL-6和IL-1β水平,其敏感性、特异性和曲线下面积(AUC)数值均较低,联合检测后,其敏感性、特异性和曲线下面积(AUC)数值均有大幅度提升.结论 联合检测HA和炎性因子水平,有助于提高OA的诊断效能,同时也为OA个性化治疗提供良好的参考价值.
Diagnostic value of combined detection of hyaluronic acid and inflammatory factors in joint fluid of osteoarthritis
Objective To explore the diagnostic value of combined detection of hyaluronic acid(HA)and inflammatory factors in joint fluid for osteoarthritis(OA).Methods A total of 102 OA patients admitted to Jiangyin Hospital of Traditional Chinese Medicine from January 2022 to January 2024 were selected as the ob-servation group,and another 102 healthy individuals undergoing physical examinations in the same hospital during the same period were selected as the control group.Joint fluid was collected from all subjects,and the levels of HA and various inflammatory factors(TNF-α,IL-6,IL-1β)in the joint fluid were measured using ELISA.The changes in HA,TNF-α,IL-6,and IL 1β between the two groups were compared,and the diag-nostic value of combined detection of HA and inflammatory factors in joint fluid for OA was assessed.Results The results of ELISA showed that compared with the control group,the level of HA in the observation group decreased significantly,and the levels of inflammatory factors such as TNF-α,IL-6 and IL-1β increased to varying degrees.The differences between the two groups were statistically significant(P<0.01).The re-sults of multivariate stepwise Logistic regression analysis showed that HA(OR=0.912,95%CI:0.858~0.971)was a protective factor for OA(P<0.01).TNF-α(OR=1.139,95%CI:1.060~1.224),IL-6(OR=1.110,95%CI:1.069~1.154)and IL-1β(OR=1.118,95%CI:1.074~1.165)were risk factors for OA(P<0.01).The results of ROC curve analysis showed that the sensitivity,specificity,and area under the curve(AUC)values were low when detecting HA,TNF-α,IL-6,and IL-1β levels individually.After com-bined detection,the sensitivity,specificity,and AUC values were significantly improved.Conclusion Com-bined detection of HA and inflammatory factor levels helps to improve the diagnostic efficiency of OA and pro-vide a good reference value for personalized treatment of OA.
