限制性液体复苏对急诊多发伤合并休克患者血清乳酸、凝血功能及预后的影响

摘要
目的 探讨限制性液体复苏在急诊多发伤合并休克中的应用效果.方法 98例急诊多发伤合并休克患者随机分为两组,对照组采用积极性液体复苏治疗,观察组采用限制性液体复苏治疗.比较两组的血清乳酸水平、pH值、凝血功能及预后效果.结果 治疗后,观察组的乳酸水平、PT、APTT均低于对照组,pH值高于对照组,多器官功能衰竭、呼吸窘迫发生率以及死亡率均低于对照组(P<0.05).结论 限制性液体复苏治疗急诊多发伤合并休克患者可有效调节血液酸度,改善凝血功能,改善预后.
Abstract
Objective To explore the application effect of limited fluid resuscitation in emergency multiple traumas complicated with shock.Methods 98 patients with emergency multiple traumas complicated with shock were randomly divided into two groups.The control group was treated with positive fluid resuscitation,and the observation group was treated with limited fluid resuscitation.The serum lactic acid level,pH value,coagulation function and prognosis effect were compared between the two groups.Results After treatment,the lactic acid level,PT and APTT in the observation group were lower than those in the control group,pH value was higher than that in the control group,and the incidences of multiple organ failure and respiratory distress and the mortality rate were lower than those in the control group(P<0.05).Conclusions Limited fluid resuscitation in the treatment of patients with emergency multiple traumas complicated with shock can effectively regulate blood acidity,and improve coagulation function and prognosis.
