目的 分析MR-HSG在不孕症女性输卵管通畅度诊断中的应用价值.方法 选取2021年1月至2022年12月我院收治的102例不孕症患者,均行MR-HSG检查.以X线下输卵管造影结果为金标准,分析MR-HSG诊断输卵管通畅度的价值.结果 102例患者经X线下输卵管造影检出输卵管阻塞40例,输卵管通畅62例,输卵管阻塞率为39.22%(40/102);经MR-HSG检出输卵管阻塞39例,输卵管阻塞检出率为98.50%(39/40).MR-HSG诊断输卵管通畅度的灵敏度为95.00%(38/40)、特异度为 98.39%(61/62)、准确度为 97.06%(99/102)、阳性预测值为 97.44%(38/39)、阴性预测值为 96.83%(61/63);Kappa检验显示,MR-HSG与X线下输卵管造影的一致性极好(Kappa值=0.938,P=0.000).结论 MR-HSG在不孕症女性输卵管通畅度诊断中具有较高的价值,可准确检出输卵管阻塞,与X线下输卵管造影一致性高,有助于早期明确诊断和治疗.
Application of MR-HSG in the Diagnosis of Tubal Patency of Infertile Women
Objective To analyze the application value of MR-HSG in the diagnosis of tubal patency of infertile women.Methods A total of 102 patients with infertility admitted to our hospital from January 2021 to December 2022 were selected,and all cases received MR-HSG examination.The value of MR-HSG in the diagnosis of tubal patency was analyzed with the results of X-ray salpingography as the gold standard.Results Among 102 patients,40 cases of fallopian tube obstruction and 62 cases of unobstructed fallopian tubes were detected by X-ray salpingography,and the fallopian tube obstruction rate was 39.22%(40/102).39 cases of fallopian tube obstruction were detected by MR-HSG,and the detection rate of fallopian tube obstruction was 98.50%(39/40).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of MR-HSG in diagnosing tubal patency were 95.00%(38/40),98.39%(61/62),97.06%(99/102),97.44%(38/39)and 96.83%(61/63),respectively.Kappa test showed that the consistency between MR-HSG and X-ray salpingography was excellent(Kappa value=0.938,P=0.000).Conclusions MR-HSG has high value in the diagnosis of tubal patency of infertile women.It can accurately detect tubal obstruction and has high consistency with X-ray salpingography,which is helpful for early diagnosis and treatment.
