血清PⅢNP、TIMP-1及左心室射血分数联合检测对顽固性心衰的诊断价值

摘要
目的 探讨血清PⅢNP、TIMP-1、左心室射血分数(LVEF)联合检测对顽固性心衰的诊断价值.方法 80 例顽固性心衰患者根据NYHA心功能分级分为Ⅲ级组和Ⅳ级组,选取同期 40 名健康者作为对照组.对比三组的血清PⅢNP、TIMP-1以及LVEF,绘制ROC曲线分析血清PⅢNP、TIMP-1 及LVEF联合检测对顽固性心衰的诊断效能.结果 Ⅳ级组、Ⅲ级组、对照组的血清PⅢNP水平依次降低,血清TIMP-1 及LVEF水平依次升高,差异有统计学意义(P<0.05).ROC曲线显示,血清PⅢNP、TIMP-1 及LVEF联合检测诊断顽固性心衰的AUC为 0.892,高于各指标单独诊断(P<0.05).结论 血清PⅢNP、TIMP-1 及LVEF对评价顽固性心衰患者的心功能具有重要价值,且三项指标联合检测可提升对顽固性心衰的诊断效能.
Abstract
Objective To explore the value of combined detection of serum PⅢNP,TIMP-1 and left ventricular ejection fraction(LVEF)in the diagnosis of refractory heart failure.Methods According to NYHA cardiac function classification,80 patients with refractory heart failure were divided into gradeⅢgroup and gradeⅣgroup;40 healthy subjects were selected as control group.The serum PⅢNP,TIMP-1 and LVEF of the three groups were compared,and the ROC curve was drawn to analyze the diagnostic efficacy of combined detection of serum PⅢNP,TIMP-1 and LVEF for refractory heart failure.Results The levels of serum PⅢNP in gradeⅣgroup,gradeⅢgroup and control group decreased in turn,and the levels of serum TIMP-1 and LVEF increased in turn,with statistically significant differences(P<0.05).ROC curve showed that the AUC of combined detection of serum PⅢNP,TIMP-1 and LVEF in the diagnosis of refractory heart failure was 0.892,higher than that of each index alone(P<0.05).Conclusions Serum PⅢNP,TIMP-1 and LVEF are of great value in evaluating cardiac function of patients with refractory heart failure,and the combined detection of the three indicators can improve the diagnostic efficacy of refractory heart failure.
