儿童腹盆部CT辐射剂量指标的比较及回归方程的建立与分析

目的 比较容积CT剂量指数(Volume CT Dose Index,CTDIvol)与基于水当量直径(Water Equivalent Diameter,WED)的体型特异性剂量估算值(Size-Specific Dose Estimate,SSDE),即SSDEWED,在儿童腹盆部辐射剂量衡量上的差异,探讨基于年龄预计算CTDIvol、SSDEWED的可行性.方法 回顾性收集2021年1-12月行腹盆部检查的812例患儿CT影像学资料,记录其年龄、CTDIvol,测量感兴趣区(Region of Interest,ROI)面积(Area of ROI,AROI)及CT值(CT Value of ROI,CTROI)并计算SSDEWED,比较CTDIvol和SSDEWED在衡量辐射剂量上的差异;分析CTDIvol、SSDEWED与年龄间相关性并建立回归方程,同时分析回归方程的一致性.结果 812例患儿腹盆部CTDIvol为(2.90±1.20)mGy,SSDEWED为(5.64±1.90)mGy,二者间差异有统计学意义(P﹤0.001).CTDIvol、SSDEWED随着年龄的增加而增加,指数函数很好地拟合了CTDIvol、SSDEWED与年龄间的相关性.实测CTDIvol为(2.90±1.20)mGy,计算CTDIvol为(2.91±1.05)mGy,二者间差异无统计学意义(t=0.97,P=0.33);实测SSDEWED为(5.64±1.90)mGy,计算SSDEWED为(5.58±1.35)mGy,二者间差异亦无统计学意义(t=1.83,P=0.07).基于年龄计算的CTDIvol、SSDEWED与各实测值间均具有强相关性(相关系数分别为0.88、0.81).结论 相较于CTDIvol,SSDEWED更能准确衡量儿童腹盆部辐射剂量,使用年龄来预计算患儿的辐射剂量指标,可以在检查前快速预估其辐射,对于辐射剂量更好的控制具有重要作用.
Comparison of CT Radiation Dose Index in Abdomen and Pelvis of Children and Establishment and Analysis of Regression Equations
Objective To compare the difference of volume CT dose index(CTDIvol)and size-specific dose estimate(SSDE)based on water equivalent diameter(WED)(that is SSDEWED)in measuring radiation dose in abdomen and pelvis of children,and to explore the feasibility of calculating CTDIvol and SSDEWED based on age.Methods The CT imaging data of 812 children who underwent abdomen and pelvis examinations from January to December of the 2021 were retrospectively collected.The age,CTDIvol were recorded;Area of Interest(AROI)and CT Value of ROI(CTROI)were measured,and SSDEWED were calculated to compare the differences in measured radiation dose between CTDIvol and SSDEWED.The correlation between CTDIvol,SSDEWED and age was analyzed,and the regression equations were generated,and then analyzed the consistency of the regression equations.Results The CTDIvol was(2.90±1.20)mGy and the SSDEWED was(5.64±1.90)mGy in 812 cases,and the difference was statistically significant(P﹤0.001).CTDIvol and SSDEWED increased with the increase of age,and the exponential function fitted well the correlation between CTDIvol,SSDEWED and age.The measured CTDIvol was(2.90±1.20)mGy,the calculated CTDIvol was(2.91±1.05)mGy,and there was no statistically significant difference(t=0.97,P=0.33);The measured SSDEWED was(5.64±1.90)mGy,the calculated SSDEWED was(5.58±1.35)mGy,and the difference was also no statistically significant(t=1.83,P=0.07).The age-based CTDIvol and SSDEWED had a strong correlations with the measured value(The correlation coefficients were 0.88 and 0.81,respectively).Conclusion Compared to CTDIvol,SSDEWED is more accurate in measuring the radiation dose in the abdomen and pelvis regions of children.Using age to predict the radiation dose index of a child can quickly estimate their radiation before examination,which plays an important role in better control of radiation dose.
