摘要
目的 探讨两种改良手术治疗上睑内翻倒睫对睑板腺功能的影响.方法 选取2019年6月～2022年6月于我院确诊为上睑内翻倒睫的80例患者作为研究对象,采用随机数字表法分组,对照组(40例)采用睑板楔形切除术(Hotz)治疗,研究组(40例)采用带蒂眼轮匝肌桥状瓣转移术治疗,比较两组患者的眼表疾病指数(OSDI)及泪膜破裂时间(BUT)、睑板腺功能情况评分、并发症发生率.结果 治疗后,两组OSDI评分均下降,且研究组低于对照组,两组BUT均提高,且研究组高于对照组(P＜0.05).治疗后,两组睑板腺功能评分均上升,研究组在术后1周、1个月、2个月均高于对照组(P＜0.05).研究组术后并发症发生率为5.00%,显著低于对照组的20.00%(P＜0.05).结论 带蒂眼轮匝肌桥状瓣转移术治疗上睑内翻倒睫的效果优于Hotz术式治疗效果,可提升患者的眼表功能,改善睑板腺功能,安全性高,并发症发生少,有利于改善患者的眼部健康.
Abstract
Objective To investigate the effects of two modified surgeries on meibomian gland function in the treatment of upper eyelid entropion trichiasis.Methods 80 patients diagnosed with upper eyelid entropion trichiasis in our hospital from June 2019 to June 2022 were selected as the research objects and grouped by random number table method.The control group(40 cases)was treated with meibomian wedge resection(Hotz),and the study group(40 cases)was treated with pedicled orbicularis oculi muscle bridge flap transfer.The ocular surface disease index(OSDI),tear film break up time(BUT),meibomian gland function score and complication rate were compared between the two groups.Results After treatment,the OSDI scores of the the two groups decreased,and that of the study group was lower than the control group.The BUT of the two groups increased,and that of the study group was higher than the control group(P<0.05).After treatment,the meibomian gland function scores of the two groups increased,and the study group was higher than the control group at 1 week,1 month and 2 months after operation(P<0.05).The incidence of postoperative complications in the study group was 5.00%,which was significantly lower than 20.00%in the control group(P<0.05).Conclusion The effect of pedicled orbicularis oculi muscle bridge flap transfer in the treatment of upper eyelid entropion trichiasis is better than that of Hotz operation.It can improve the ocular surface function and meibomian gland function of patients,with high safety and few complications,which is conducive to improving the eye health of patients.
