摘要
目的 探究肾功能指标联合CRP、ET-1与慢性心力衰竭患者房颤发生率的相关性.方法 选取我院2020年1月~2023年1月纳入的86例慢性心力衰竭患者,根据是否伴有房颤分为慢性心衰合并房颤组和慢性心衰组(n=43).观察两组患者血清胱抑素C(CysC)、肌酐(Cr)、尿素氮(BUN)等肾功能指标,以及血管内皮素(ET-1)、C反应蛋白(CRP)水平的差异,分析CysC、Cr、BUN、ET-1、CRP水平与慢性心衰患者发生房颤的相关性.结果 两组患者ET-1水平、CRP水平、CysC水平、年龄、合并吸烟史、合并冠心病史、合并高血压史方面,差异具有统计学意义(P<0.05);慢性心力衰竭合并房颤与肾功能指标CysC(r=0.419,P<0.01)和ET-1(r=0.209,P<0.01)、CRP(r=0.228,P<0.01)等因素相关,但与高血压、吸烟、BUN等因素无关.Logistic回归分析结果也显示,ET-1、CRP、CysC水平为慢性心衰患者发生房颤的危险因素.结论 血清CysC、ET-1、CRP水平与慢性心力衰竭患者房颤密切相关,且上述指标上升的患者房颤较为多发.
Abstract
Objective To explore the correlation between renal function indexes combined with CRP,ET-1 and the incidence of atrial fibrillation in patients with chronic heart failure.Methods A total of 86 patients with chronic heart failure enrolled in our hospital from January 2020 to January 2023 were selected and divided into two groups:chronic heart failure with atrial fibrillation group and chronic heart failure group(n=43).The differences of serum cystatin C(Cys),creatinine(Cr),urea nitrogen(BUN)and other renal function indexes,vascular endothelin(ET-1)and C-reactive protein(CRP)levels between the two groups were observed,and the correlation between CysC,Cr,BUN,ET-1 and CRP levels and the occurrence of atrial fibrillation in patients with chronic heart failure was analyzed.Results There were statistically significant differences in ET-1 level,CRP level,CysC level,age,smoking history,coronary heart disease history and hypertension history between the two groups(P<0.05).Chronic heart failure with atrial fibrillation was correlated with renal function index CysC(r=0.419,P<0.01),ET-1(r=0.209,P<0.01),CRP(r=0.228,P<0.01),but not with hypertension,smoking history,BUN and other factors.Logistic regression analysis also showed that ET-1,CRP and CysC levels were risk factors for atrial fibrillation in patients with chronic heart failure.Conclusion Serum renal function index CysC,ET-1 and CRP levels are closely related to atrial fibrillation in patients with chronic heart failure,and patients with increased levels of these indexes are more prone to atrial fibrillation.
